Formoterol Nebulizer Dosing in Adults
The recommended dose of formoterol nebulizer solution in adults is 20 mcg administered twice daily (morning and evening) via nebulization, with a maximum total daily dose of 40 mcg. 1

Critical Indication Restrictions

- Use of any long-acting beta-agonist (LABA) without an inhaled corticosteroid is contraindicated in asthma patients due to increased risk of asthma-related death 1

- Validated delivery system: PARI-LC Plus nebulizer with PRONEB Ultra compressor 1
- Non-compressor based nebulizer systems have not been established as safe or effective 1
- Store vials in foil pouch and remove IMMEDIATELY before use 1

Important Safety Warnings
Black Box Warning: LABAs, including formoterol, increase the risk of asthma-related hospitalizations, intubations, and death when used as monotherapy without inhaled corticosteroids. 2, 1
- This is considered a class effect of all LABAs 1
- Formoterol nebulizer solution should NOT be used to treat acute deteriorations of COPD 1
- If maintenance treatment fails to provide usual response, seek medical advice immediately as this indicates COPD destabilization 1
Common Pitfalls to Avoid
- Do not confuse nebulizer dosing (maximum 40 mcg/day) with dry powder inhaler dosing (maximum 24 mcg/day for maintenance or 54 mcg/day in SMART protocols) 2, 1, 3
- Do not mix formoterol nebulizer solution with other drugs in the nebulizer - compatibility has not been established 1
