Available Formulations
Tablet Form
- Fluconazole tablets are available in 50 mg, 100 mg, 150 mg, and 200 mg strengths for oral administration 1
- Tablets are the most commonly prescribed formulation for adults with oral thrush 2
- Oral fluconazole tablets are considered the drug of choice for oropharyngeal candidiasis due to superior efficacy and convenience compared to topical therapy 2
Liquid Form (Oral Suspension)
- Fluconazole oral suspension is specifically available for pediatric patients and adults who cannot swallow tablets 3
- The suspension formulation demonstrated 91% clinical cure rates in immunocompromised children with oral thrush, significantly superior to nystatin (51% cure rate) 3
- Oral solution formulations provide both local mucosal effects and systemic absorption 2
Clinical Dosing by Formulation
For Adults
- Standard tablet dosing: 100-200 mg daily for 7-14 days 2
- Single-dose therapy with 150 mg tablet showed 96.5% improvement in palliative care patients 4
- Tablets achieve therapeutic concentrations rapidly with excellent bioavailability exceeding 93% 5
For Pediatric Patients
- Suspension dosing: 2-3 mg/kg per day as a single daily dose for 14 days 3
- The liquid formulation is better tolerated in children who may have difficulty with troches or tablets 3
Important Clinical Considerations
Both formulations have equivalent efficacy when dosed appropriately, as absorption and bioavailability are excellent regardless of formulation 5, 6
- Food intake does not affect absorption of either formulation 5
- Peak plasma concentrations occur 2.4-3.7 hours after oral administration of either tablets or suspension 5
- The long half-life (31-37 hours) allows for once-daily dosing with both formulations 5, 6
Common Pitfalls to Avoid
- Do not assume liquid formulations are only for children—adults with swallowing difficulties or severe oral pain may benefit from suspension 3
- Avoid confusing fluconazole oral formulations with topical agents like clotrimazole troches or nystatin suspension, which are different medications 7
- For moderate-to-severe oral thrush, systemic fluconazole (tablet or suspension) is superior to topical therapy and should be used preferentially 2, 7
