Levocetirizine Dosing: Maximum Therapeutic Dose
The standard therapeutic dose of levocetirizine is 5 mg once daily, but doses can be safely increased up to 20 mg daily (4 times the conventional dose) in patients with difficult-to-treat allergic conditions without compromising safety. 1
Standard Dosing
- The approved dose of levocetirizine is 5 mg once daily for treatment of allergic rhinitis (including persistent allergic rhinitis) and chronic idiopathic urticaria 2, 3
- This standard 5 mg dose provides rapid onset of action with sustained 24-hour symptom control 4, 5
- Levocetirizine at 5 mg daily significantly improves nasal symptoms, including nasal congestion, as early as the first week of treatment 4
Dose Escalation for Refractory Cases
When standard doses fail to provide adequate symptom control, levocetirizine can be safely escalated up to 4-fold (20 mg daily) based on high-quality evidence. 1
- In a randomized, double-blind trial of 80 patients with difficult-to-treat chronic urticaria, doses were escalated weekly from 5 mg to 10 mg to 20 mg if symptom relief remained incomplete 1
- Only 13 patients (approximately 15%) achieved complete symptom control at the standard 5 mg dose 1
- An additional 28 patients became symptom-free when doses were increased to 10 mg or 20 mg daily 1
- Approximately 75% of patients with difficult-to-treat urticaria responded to higher-than-conventional antihistamine doses (up to 4-fold), while only 10% were complete non-responders 1
Safety Profile at Higher Doses
Critically, increasing levocetirizine doses up to 4-fold (20 mg daily) did not increase somnolence or cause serious adverse effects. 1

Clinical Algorithm for Dose Titration
Start with 5 mg once daily and assess response after 1 week: 1, 4
- If symptoms resolve completely, continue 5 mg daily
- If partial response or persistent symptoms, increase to 10 mg daily
- Reassess after 1 week at 10 mg dose
- If still inadequate control, increase to 20 mg daily
- Maximum therapeutic dose is 20 mg daily (4 times conventional dose)
Important Caveats
- The evidence for dose escalation comes primarily from chronic urticaria patients; extrapolation to other allergic conditions should be done cautiously 1
- Individual heterogeneity in antihistamine responsiveness is substantial—some patients are excellent responders at low doses while others require maximum doses 1
- Long-term studies demonstrate sustained efficacy for up to 6 months at standard dosing in persistent allergic rhinitis 4
