Clinical Activity Assessment in Outpatient Crohn's Disease
For outpatient assessment of Crohn's disease activity, use the Harvey-Bradshaw Index (HBI), which is simple, practical, and specifically designed for clinical practice rather than research. 1
Why the Harvey-Bradshaw Index (HBI)
The HBI is the preferred clinical tool for outpatients because it:
- Requires only clinical parameters from the previous day (general well-being, abdominal pain, number of liquid stools, abdominal mass, and complications) 1
- Takes minutes to calculate without requiring a 7-day patient diary 1
- Defines remission as HBI ≤4, making it straightforward to interpret 1
- Is validated and reproducible for routine clinical use 1
Why NOT the CDAI for Outpatients
While the Crohn's Disease Activity Index (CDAI) is the most widely used measure in clinical trials, it is explicitly not recommended for routine outpatient practice due to: 1
- Requires a cumbersome 7-day symptom diary from patients 1
- Complex calculation with eight weighted variables 1, 2
- Heavy weighting toward diarrhea (which may be caused by factors other than inflammation) 1
- Not usable in patients with stomas 1
- Not validated for postoperative use 1
- Underestimates fistulizing or stricturing disease 1
Important Caveats About Clinical Scores
Neither HBI nor CDAI correlates well with objective markers of inflammation (endoscopy, CRP, fecal calprotectin), as psychological factors significantly impact patient-reported symptoms. 1 Therefore:
- Clinical scores must be complemented with objective measures including fecal calprotectin, CRP, endoscopy, or cross-sectional imaging 1
- The HBI may overestimate disease activity when functional bowel symptoms coexist 1
- The HBI may underestimate disease in patients with subclinical stricturing or penetrating complications 1
Endoscopic Scoring (When Applicable)
For endoscopic assessment (not routine outpatient visits), the Simple Endoscopic Score for Crohn's disease (SES-CD) is preferred over CDEIS because: 1
- Simple, reproducible, and reliable 1
- CDEIS is too complex and time-consuming for routine practice 1
- Both are validated but SES-CD is practical 1
