Can Liver Cysts Resolve on Their Own?

Simple hepatic cysts do not typically resolve spontaneously, but intracystic hemorrhage within a hepatic cyst does resolve on its own without requiring treatment. 1

Natural History of Simple Hepatic Cysts

Simple hepatic cysts are benign developmental lesions that generally follow an indolent course without significant changes in size over time. 1 The key points about their natural behavior include:

  • Symptomatic cysts warrant volume-reducing therapy (surgical fenestration or percutaneous aspiration sclerotherapy), with treatment success defined by symptom relief, not cyst disappearance. 2, 3

Common Pitfalls to Avoid

  • Do not order surveillance imaging for asymptomatic simple cysts based on size alone, as this leads to unnecessary healthcare utilization without improving outcomes. 4
  • Do not expect cysts to disappear after treatment—treatment success is measured by symptom relief, not volume reduction or complete resolution. 1, 2, 3
  • Do not confuse simple cysts with hemorrhagic cysts—only the hemorrhagic component resolves spontaneously, not the cyst itself. 1, 2
