From the Guidelines
The standard dosage of rabeprazole for adults is typically 20 mg once daily, taken before a meal, preferably in the morning, as indicated in the study 1. For gastroesophageal reflux disease (GERD), the usual treatment duration is 4-8 weeks, while for duodenal ulcers, it's generally 4 weeks. For maintenance therapy of GERD or erosive esophagitis, a lower dose of 10 mg daily may be sufficient. Some key points to consider when prescribing rabeprazole include:
- Patients with severe hepatic impairment should consider a reduced dose.
- Rabeprazole works by decreasing stomach acid production as a proton pump inhibitor, blocking the enzyme system responsible for acid secretion in the stomach lining.
- The medication should be swallowed whole with water, not crushed or chewed, as this could destroy the enteric coating designed to protect the drug from stomach acid.
- Common side effects may include headache, nausea, and diarrhea.
- Patients should be aware that long-term use (beyond one year) may increase risks of bone fractures, vitamin B12 deficiency, and certain infections, so regular reassessment of treatment necessity is important, as suggested in the study 1. Some specific considerations for dosing include:
- A 4- to 8-week trial of single-dose PPI is considered safe and appropriate for patients with typical reflux symptoms and no alarm symptoms, with escalation to twice-a-day dosing or switching to a more potent acid suppressive agent if symptoms persist, as indicated in the study 1.
- Symptom response should prompt PPI titration to the lowest effective dose.
- When long-term PPI therapy is planned, objective reflux testing should be offered to establish a diagnosis of GERD and a long-term management plan. It's also important to note that the use of PPIs twice daily is supported by expert opinion, as indicated in the study 1, which recommends twice-daily dosing of PPIs to improve symptom relief in patients with an esophageal GERD syndrome with an unsatisfactory response to once-daily dosing.

From the Research
Dosage of Rabeprazole
- The dosage of rabeprazole for the treatment of acid-related disorders, including gastroesophageal reflux disease (GERD), duodenal ulcer, gastric ulcer, and Helicobacter pylori, has been studied in several clinical trials 3, 4, 5, 6, 7.
- For oesophageal reflux disease without erosions, both 10 and 20 mg of rabeprazole are equivalent and better than placebo at 2 and 4 weeks 3.
- An on-demand approach to non-erosive reflux disease with 10 mg of rabeprazole has also been documented as superior to placebo 3.
- Rabeprazole 10 or 20 mg once daily has been shown to relieve heartburn within the first 1 or 2 days of treatment and also has significant positive effects on other GERD symptoms, including regurgitation, belching, bloating, satiety, and nausea 4.
- The efficacy of rabeprazole once daily for acid-related disorders has been evaluated in 12 controlled clinical trials, and it has been found to be as effective as omeprazole for each indication and significantly more effective than ranitidine for healing of GERD and duodenal ulcer 5.
Specific Dosage Recommendations
- For the treatment of GERD, the recommended dosage of rabeprazole is 10 or 20 mg once daily 3, 4.
- For the treatment of non-erosive reflux disease, the recommended dosage of rabeprazole is 10 mg on-demand 3.
- For the treatment of duodenal and gastric ulcers, the recommended dosage of rabeprazole is 10 or 20 mg once daily 5, 6.
- For the eradication of Helicobacter pylori, the recommended dosage of rabeprazole is 10 or 20 mg twice daily as part of combination therapy 6.
