Clindamycin Dosing for a 25kg Child at 12mg/kg Daily
This dosing request is BELOW standard therapeutic recommendations and should NOT be used.
The requested dose of 12 mg/kg/day is substantially lower than guideline-recommended dosing for clindamycin in pediatric patients, which ranges from 25-40 mg/kg/day for most infections. 1, 2 This subtherapeutic dosing will likely result in treatment failure and should be reconsidered.

Why 12 mg/kg/day is Inadequate
The American Academy of Pediatrics and Infectious Diseases Society of America consistently recommend minimum doses of 25-30 mg/kg/day for oral therapy and 40 mg/kg/day for parenteral therapy to achieve therapeutic concentrations against common pathogens. 1, 2 Even the FDA's lowest recommended dosing for "serious infections" starts at 8-16 mg/kg/day, with 12 mg/kg/day falling at the lower end of this range and only appropriate for the least severe infections. 3
