Duration of the Infective Phase of COVID-19
For mild to moderate COVID-19, patients are infectious for 10 days from symptom onset, while severely ill patients remain infectious for 15-20 days, with peak contagiousness occurring in the 2 days before and 5 days after symptom onset. 1
Peak Infectivity Window
The highest risk of transmission occurs during a narrow 7-day window:
- Viral shedding begins 1-2 days before symptoms appear, making pre-symptomatic transmission a critical feature 2
- Peak viral loads occur within the first 7 days of illness 1, 2
- Maximum contagiousness is in the 2 days before and 5 days after symptom onset 1
- Live virus remains detectable in respiratory secretions for up to 9 days in most individuals 1
This pre-symptomatic transmission capability distinguishes COVID-19 from many other respiratory infections and explains its rapid pandemic spread 2.
Duration by Disease Severity
Mild to Moderate Disease
- Isolation required for 10 days from symptom onset 1
- Viral shedding continues for 1-2 weeks 1, 2
- It is rare to culture live virus beyond 10 days after symptom onset 1
- This 10-day recommendation aligns with UK and World Health Organization guidelines 1
Severe or Critical Disease
- Isolation required for 15-20 days from symptom onset 1
- Risk of replication-competent virus is approximately 5% at 15 days 1
- Viable virus is extremely rare at 20 days 1
- The longest documented interval with replication-competent virus is 20 days from symptom onset 3
Severely Immunocompromised Patients
- Infectivity may continue beyond standard timeframes 1
- Quarantine should extend to at least 20 days or more following symptom onset 2
- These patients require specialist advice on duration of self-isolation 1
Critical Clinical Pitfalls
PCR positivity does NOT correlate with secretion of live virus and is of little to no value in assessing infectivity risk in the 3 months after confirmed infection 1. This is a common misunderstanding that leads to unnecessarily prolonged isolation.
Approximately 30-60% of patients shedding virus may be asymptomatic, making asymptomatic transmission a significant concern 1, 2. These individuals follow the same infectivity timeline but without the symptom-onset marker to guide isolation timing.
Practical Isolation Algorithm
For symptomatic patients with mild-moderate disease:
For symptomatic patients with severe-critical disease:
- Isolate for 15-20 days from symptom onset 1
- Consider extended isolation if severely immunocompromised 2
For asymptomatic positive cases:
- Isolate for 10 days from positive test date 1
For healthcare settings and surgical procedures:
