Metronidazole Vaginal Gel Dispensing for Bacterial Vaginosis
Dispense 25 grams total of metronidazole vaginal gel 0.75% (5 applicators, each containing 5 grams) for the standard once-daily regimen, or 50 grams total (10 applicators) if using the twice-daily regimen. 1
Standard Dosing Regimens
The FDA-approved dosing is one full applicator (5 grams containing 37.5 mg metronidazole) intravaginally once or twice daily for 5 days 1. This translates to:
- Once-daily regimen (preferred): 5 grams at bedtime × 5 days = 25 grams total 2, 3, 1
- Twice-daily regimen (alternative): 5 grams twice daily × 5 days = 50 grams total 4, 3, 1
Why Once-Daily Dosing is Preferred
The once-daily regimen is equally effective to twice-daily dosing and improves patient compliance. 2 Clinical trial data demonstrates equivalent cure rates between once-daily (77% at first follow-up, 58% at one month) and twice-daily dosing (80% at first follow-up, 61% at one month), with no statistically significant difference 5. Critical Patient Counseling
- Alcohol avoidance is mandatory: Patients must avoid all alcohol consumption during treatment and for 24 hours after completion to prevent disulfiram-like reactions 4, 2, 3
- Contraceptive safety: Unlike clindamycin cream, metronidazole gel does NOT weaken latex condoms or diaphragms 3

Common Pitfall to Avoid
Never use metronidazole vaginal gel for trichomoniasis treatment—topical metronidazole is considerably less efficacious than oral preparations for trichomoniasis and should only be used for bacterial vaginosis 2.
