Intestinal Kaposi sarcoma is a rare vascular tumor that affects the gastrointestinal tract in patients with human herpesvirus 8 (HHV-8) infection, most commonly occurring in immunocompromised individuals, particularly those with HIV/AIDS, and treatment primarily involves addressing the underlying immunosuppression with antiretroviral therapy as the cornerstone for HIV-associated cases. The intestinal form typically develops after cutaneous lesions appear but can occasionally be the first manifestation of the disease. Symptoms include:

  • Abdominal pain
  • Bleeding
  • Diarrhea
  • Obstruction
  • Weight loss though many patients remain asymptomatic with lesions discovered incidentally during endoscopy. Diagnosis requires endoscopic visualization of characteristic red-purple lesions followed by biopsy confirmation. For more aggressive disease, systemic chemotherapy with liposomal doxorubicin (20-40 mg/m² every 2-3 weeks) or paclitaxel (100 mg/m² weekly) is recommended 1. Local treatments like radiation therapy may help with bleeding or obstruction. The prognosis depends largely on immune status, with better outcomes in patients who achieve immune reconstitution, as noted in the guidelines for AIDS-related Kaposi sarcoma 2, 1, 3, 4, 5, 6, 7. Regular endoscopic surveillance is important for monitoring disease progression and treatment response. It is also important to note that HHV-8 is not eradicated with treatment of Kaposi sarcoma, and the risk of future Kaposi sarcoma persists even after complete remission, highlighting the need for ongoing monitoring and management of HIV control and immune function 4.

Intestinal Kaposi Sarcoma

  • Chemotherapy options such as liposomal doxorubicin and paclitaxel are commonly used to treat aggressive and extensive forms of Kaposi sarcoma 9, 10.
  • Restoring immunity in immunocompromised patients is also an important aspect of treatment, alongside conventional local therapies 9.

Clinical Presentation and Epidemiology

  • Kaposi sarcoma can manifest in different forms, including classic, endemic, epidemic, iatrogenic, and in men having sex with men, each with distinct clinical features depending on immune status and geographic area of origin 9, 10.
  • The disease is primarily associated with Kaposi's sarcoma-associated herpesvirus (KSHV), and viral proteins can induce KS-associated cellular changes that enable the virus to evade the host immune system 10.
  • Currently, most cases of Kaposi sarcoma occur in sub-Saharan Africa, where KSHV infection is prevalent due to transmission by saliva in childhood compounded by the ongoing AIDS epidemic 10.
