Tinidazole Dosing for Gastroenteritis
For giardiasis, give tinidazole 2 g orally as a single dose with food; for intestinal amebiasis, give 2 g orally once daily for 3 days with food; and for amebic liver abscess, give 2 g orally once daily for 3-5 days with food. 1
Giardiasis Treatment
Tinidazole is the preferred first-line agent over metronidazole for giardiasis. 2

- Pediatric dose (≥3 years): 50 mg/kg (maximum 2 g) as a single dose with food 1
- This single-dose regimen achieves cure rates of 90% and offers superior tolerability compared to multi-day metronidazole courses 3

Intestinal Amebiasis Treatment
For symptomatic intestinal amebiasis caused by Entamoeba histolytica:
- Adult dose: 2 g orally once daily for 3 days with food 1
- Pediatric dose (≥3 years): 50 mg/kg/day (maximum 2 g/day) once daily for 3 days with food 1
- Tinidazole achieves significantly higher cure rates (90-96.5%) compared to metronidazole (53-55.5%) for intestinal amebiasis 5, 6
- Critical follow-up: After completing tinidazole, all patients must receive a luminal amebicide (diloxanide furoate 500 mg three times daily or paromomycin 30 mg/kg/day in 3 divided doses for 10 days) to prevent relapse, even with negative stool microscopy 7
Amebic Liver Abscess Treatment
For amebic liver abscess:
- Adult dose: 2 g orally once daily for 3-5 days with food 1
- Pediatric dose (≥3 years): 50 mg/kg/day (maximum 2 g/day) once daily for 3-5 days with food 1
- Tinidazole 2 g daily for 3 days results in less nausea compared to metronidazole 500 mg three times daily for 7-10 days 7
- Most patients respond within 72-96 hours 7
- Children should be closely monitored when treatment duration exceeds 3 days, as pediatric data beyond 3 days are limited 1
- After completing tinidazole, all patients require a luminal amebicide to reduce relapse risk 7
Critical Administration Instructions
- Take tinidazole with food to minimize gastrointestinal side effects 1
- Avoid alcoholic beverages during treatment and for 3 days after completion 1
- For patients unable to swallow tablets, crush four 500 mg tablets in 30 mL artificial cherry syrup (stable for 7 days at room temperature; shake well before each use) 1
Common Pitfalls to Avoid
- Do not use tinidazole alone for amebic infections without follow-up luminal amebicide therapy, as this increases relapse risk 7
