The pharmacokinetic profile supports once-daily dosing at any time. Ezetimibe has a terminal elimination half-life of approximately 22 hours, with peak concentrations reached 1-2 hours post-administration followed by enterohepatic recycling and slow elimination, allowing flexible timing without compromising efficacy. 3
Food does not affect ezetimibe absorption. The medication can be administered with or without food, further supporting flexible timing based on patient preference and adherence patterns. 1, 3
Practical Dosing Recommendations
Standard dose is 10 mg once daily, taken at the same time each day to optimize adherence. 1
Pair ezetimibe timing with an established daily routine (e.g., morning coffee, evening tooth brushing) to maximize medication adherence, which is more important than the specific clock time chosen. 2
If taking bile acid sequestrants concurrently, administer ezetimibe either ≥2 hours before or ≥4 hours after the bile acid sequestrant to avoid reduced bioavailability, as cholestyramine significantly decreases ezetimibe absorption. 1, 3
Common Pitfalls to Avoid
Do not instruct patients that evening dosing is mandatory (as is sometimes recommended for statins due to circadian cholesterol synthesis), since ezetimibe works by blocking intestinal cholesterol absorption rather than hepatic synthesis, making timing irrelevant to efficacy. 2
