Tadalafil Treatment Guidelines
Erectile Dysfunction
For erectile dysfunction, start with tadalafil 10 mg taken as needed 30 minutes before sexual activity, with dose adjustment to 20 mg or 5 mg based on response and tolerability. 1
As-Needed Dosing
- Starting dose: 10 mg taken at least 30 minutes before anticipated sexual activity 1
- Titrate to 20 mg if inadequate response, or reduce to 5 mg if side effects occur 1
- Maximum dosing frequency is once per day 1
- Effective for up to 36 hours after dosing, allowing spontaneous sexual activity without precise timing 1, 2
- Tadalafil does NOT show dose-dependent improvement between 5 mg, 10 mg, and 20 mg—choose between doses based on tolerability rather than expecting dramatically better efficacy at higher doses 2, 3
Daily Dosing Alternative
- Starting dose: 2.5 mg once daily at the same time each day, without regard to timing of sexual activity 1
- May increase to 5 mg daily based on efficacy and tolerability 1
- Daily dosing eliminates the need to time medication with sexual activity, providing an alternative for couples who prefer spontaneous rather than scheduled sexual encounters 2
Efficacy Data
- 69% of men using tadalafil as needed report successful sexual intercourse attempts compared to 33% with placebo 2
- Sexual stimulation is necessary for tadalafil to be effective—incorrect use accounts for a large percentage of treatment failures 2
- An adequate trial requires at least 5 separate attempts at maximum tolerated dose before declaring treatment failure 2
Common Pitfalls to Avoid
- Before declaring treatment failure, systematically evaluate: inadequate sexual stimulation, improper timing, heavy alcohol use, hormonal abnormalities, and relationship issues 2, 4
- Men with diabetes and post-prostatectomy patients have more severe ED at baseline and may respond less robustly to PDE5 inhibitors 2
Benign Prostatic Hyperplasia (BPH)
For BPH, use tadalafil 5 mg once daily taken at approximately the same time every day. 1
Dosing
- Fixed dose: 5 mg once daily 1
- No dose titration for BPH indication 1
- When initiating therapy with both tadalafil and finasteride, use tadalafil 5 mg daily for up to 26 weeks 1
Efficacy
- Tadalafil 5 mg once daily significantly improves International Prostate Symptom Score (IPSS) compared to placebo 5
- Improvements in IPSS maintained over 1-year treatment period 5
Combined ED and BPH
For men with both erectile dysfunction and BPH, tadalafil 5 mg once daily treats both conditions simultaneously—this is FDA-approved. 1, 2
Dosing
- Single dose: 5 mg once daily at approximately the same time each day, without regard to timing of sexual activity 1
- This approach is more convenient than using separate medications for each condition 2
Efficacy
- Both erectile function (IIEF-EF domain) and BPH symptoms (total IPSS) significantly improved from baseline to week 12 with tadalafil 5 mg daily versus placebo 5
- Tadalafil 5 mg once daily significantly improved ejaculation, orgasm, intercourse satisfaction, overall satisfaction, and erectile function in men with both conditions 6
Pulmonary Arterial Hypertension (PAH)
For treatment-naive PAH patients with WHO Functional Class II or III symptoms, tadalafil is suggested to improve 6-minute walk distance, WHO functional class, delay time to clinical worsening, and improve cardiopulmonary hemodynamics. 7
Dosing
- Maximum approved dose: 40 mg once daily for PAH 7, 3
- Treatment with more than 40 mg once daily has not been studied and is not recommended 7
- The higher dosing for PAH (40 mg) versus ED (maximum 20 mg as-needed or 5 mg daily) reflects the life-threatening nature of PAH justifying more aggressive treatment despite potential side effects 3
Evidence Strength
- Grade 2C recommendation for improving 6-minute walk distance in WHO FC III patients 7
- Grade 2C recommendation for improving WHO FC in WHO FC II patients 7
- Grade 2C recommendation for delaying time to clinical worsening 7
Critical Drug Interaction
- Male patients treated with riociguat for PAH should be cautioned NOT to use PDE5 inhibitors (including tadalafil) for erectile dysfunction due to risk of systemic hypotension 7
Critical Safety Considerations
Absolute Contraindications
- Tadalafil is absolutely contraindicated with nitrates in any form due to risk of potentially fatal hypotension 1, 2
- Patients must be explicitly asked about all forms of nitrate use before prescribing 2
Cardiovascular Risk Stratification Required
- Before prescribing tadalafil, assess whether the patient can perform moderate physical activity (walk 1 mile in 20 minutes or climb 2 flights of stairs in 20 seconds) 2
- Refer to cardiology if unable to perform this level of activity 2
- Low-risk patients (asymptomatic CAD with <3 risk factors, controlled hypertension, mild stable angina, successful coronary revascularization, uncomplicated past MI, mild valvular disease, CHF NYHA class I) can safely receive tadalafil 2
- High-risk patients should defer treatment until cardiac condition stabilized 7
Emergency Nitrate Protocol
- If a patient on tadalafil requires emergency nitrate administration, wait 48 hours after tadalafil before giving nitrates, under close medical supervision with patient monitoring 2
Common Adverse Effects
- Headache, dyspepsia, back pain, myalgia, nasal congestion 2, 3
- Tadalafil has lower rates of flushing compared to other PDE5 inhibitors 2
Serious Adverse Events
- Sudden vision loss: Stop tadalafil immediately and seek medical attention—may indicate NAION (non-arteritic anterior ischemic optic neuropathy) 1
- Sudden hearing loss: Stop tadalafil immediately and seek prompt medical attention 1
- Priapism: Erection lasting >4 hours requires emergency medical attention 1
Alcohol Interaction
- Heavy alcohol use (≥5 units) combined with tadalafil increases risk of orthostatic hypotension, dizziness, and headache 1
- Heavy alcohol use should be addressed before declaring tadalafil therapy ineffective 2
Dose Adjustments for Special Populations
Renal Impairment - As-Needed Dosing
- CrCl 30-50 mL/min: Start with 5 mg, maximum 10 mg once every 48 hours 1
- CrCl <30 mL/min or hemodialysis: Maximum 5 mg once every 72 hours 1
Renal Impairment - Daily Dosing
- CrCl <30 mL/min or hemodialysis: Daily tadalafil NOT recommended for ED 1
- CrCl 30-50 mL/min for BPH or ED/BPH: Start with 2.5 mg daily, may increase to 5 mg based on response 1
- CrCl <30 mL/min or hemodialysis: Daily tadalafil NOT recommended for BPH or ED/BPH 1
Hepatic Impairment - As-Needed Dosing
- Mild or moderate (Child-Pugh Class A or B): Maximum 10 mg once per day 1
- Severe (Child-Pugh Class C): Tadalafil NOT recommended 1
Hepatic Impairment - Daily Dosing
- Mild or moderate (Child-Pugh Class A or B): Use with caution, not extensively evaluated 1
- Severe (Child-Pugh Class C): Tadalafil NOT recommended 1
Concomitant Medications
Alpha-Blockers
- Patients should be stable on alpha-blocker therapy before initiating tadalafil for ED 1
- Start tadalafil at the lowest recommended dose when coadministered with alpha-blockers 1
Combination with Testosterone
- For men with ED and testosterone deficiency (total testosterone <230 ng/dL or 230-350 ng/dL), combining tadalafil with testosterone therapy may be more effective than tadalafil alone 2
- Testosterone therapy alone is NOT effective monotherapy for ED 2
- Check morning serum total testosterone in all men presenting with ED, as low testosterone may explain poor response to PDE5 inhibitors 4

- Tadalafil may be taken without regard to food 1
- For as-needed dosing, take at least 30 minutes before anticipated sexual activity 1
