Terbinafine should be prescribed for 6 weeks for fingernail fungal infections and 12-16 weeks for toenail fungal infections, as this duration has been shown to be effective in achieving a complete cure and preventing relapse 1. The standard adult oral dose is 250 mg once daily, and treatment duration is crucial because fungal nail infections require sufficient time for the medication to penetrate the nail bed and eliminate the infection as the nail grows out.
- The medication works by inhibiting the enzyme squalene epoxidase, which disrupts fungal cell membrane synthesis, making it particularly effective against dermatophytes, the most common cause of nail infections.
- Patients should take the full prescribed course even if improvement is seen earlier, and they should be aware that complete nail appearance may not normalize until the infected portion grows out completely, which can take several months after completing treatment.
- According to the British Association of Dermatologists' guidelines, terbinafine is the preferred treatment for dermatophyte onychomycosis due to its higher efficacy and tolerability compared to itraconazole 1.
- The guidelines also recommend monitoring liver function tests and complete blood count in adult patients with a history of hepatotoxicity or hematological abnormalities, as terbinafine can cause headache, taste disturbance, and gastrointestinal upset, and can aggravate psoriasis and cause a subacute lupus-like syndrome 1.
