Administration Guidelines for Adults
The FDA-approved label specifies two acceptable administration methods for adults:
- 30-minute IV infusion: The standard and preferred method for adult patients 1
- 2-minute IV injection: An alternative rapid administration option available only for adults 1
For the 30-minute infusion, the reconstituted daptomycin (50 mg/mL concentration) should be further diluted into a 50 mL IV infusion bag containing 0.9% sodium chloride injection 1. The infusion rate should be maintained at 1.67 mL/minute over the 30-minute period 1.
Critical Reconstitution Instructions
To minimize foaming during preparation, avoid vigorous agitation or shaking of the vial during or after reconstitution 1. The reconstituted solution must be prepared using only 0.9% sodium chloride injection, as daptomycin is not compatible with dextrose-containing diluents 1.
Pediatric Considerations (Not Applicable Here, But Important to Note)
Do NOT administer daptomycin by 2-minute injection to pediatric patients 1. Pediatric patients ages 7-17 years require 30-minute infusions, while those ages 1-6 years require 60-minute infusions 1.
Common Pitfalls to Avoid
- Never use dextrose-containing solutions for reconstitution or dilution, as daptomycin is incompatible with these diluents 1
- Do not use ReadyMED® elastomeric infusion pumps with daptomycin, as stability studies have shown incompatibility 1
- Avoid vigorous shaking during reconstitution to prevent excessive foaming 1
- The combined storage time of reconstituted and diluted solutions should not exceed 12 hours at room temperature or 48 hours under refrigeration 1
