Mechanism of Concern
The traditional concern stems from a potential disulfiram-like reaction, where metronidazole may inhibit aldehyde dehydrogenase, leading to acetaldehyde accumulation when alcohol is consumed. 3 Acetaldehyde is hepatotoxic, cardiotoxic, and arrhythmogenic, and at least one case report documents sudden death attributed to cardiac dysrhythmia from acetaldehyde toxicity due to metronidazole-ethanol interaction. 3
Evidence Quality and Controversy
Important caveat: While official guidelines uniformly recommend alcohol avoidance, the actual clinical evidence supporting a significant interaction is surprisingly limited. 4 A 2014 systematic review found no convincing evidence from in-vitro studies, animal models, adverse effect reports, or clinical studies demonstrating a clinically relevant disulfiram-like interaction between ethanol and metronidazole. 4 The authors concluded that reported reactions were equally likely caused by ethanol alone or by adverse effects of metronidazole itself. 4
Clinical Recommendation
