MenB Vaccines Are NOT Interchangeable
The two MenB vaccines—MenB-FHbp (Trumenba) and MenB-4C (Bexsero)—are not interchangeable, and the same vaccine product must be used for all doses in a series, including booster doses. 1, 2, 3

The two vaccines contain different antigens and have distinct mechanisms of protection:
- MenB-FHbp (Trumenba) contains two variants of factor H binding protein 3
- MenB-4C (Bexsero) contains four components including outer membrane vesicles and three recombinant proteins 1
Because efficacy has not been established for persons receiving MenB vaccines interchangeably, every effort should be made to determine which vaccine product was used for all received doses 1.
What To Do In Practice
If You Know Which Product Was Started
- Complete the series with the same product 1, 2
- Document the specific product name (not just "MenB") in the patient's record 3
If the Original Product Is Unknown
The guidelines do not provide specific recommendations for this scenario, but the FDA label explicitly states that data are not available on the safety and effectiveness of using Trumenba and other meningococcal group B vaccines interchangeably 3. In real-world practice, you must:
- Make every effort to identify the original product through immunization registries, previous providers, or pharmacy records 1
- If truly unknown and the patient is at high risk (complement deficiency, asplenia, outbreak exposure), consider restarting the series with a documented product rather than mixing vaccines
- For healthy adolescents receiving MenB under shared clinical decision-making, document the discussion about unknown prior product and proceed with a complete series of one product
Dosing Schedules (Product-Specific)
MenB-FHbp (Trumenba) 1, 3
- High-risk patients: 3 doses at 0,1-2, and 6 months
- Healthy adolescents: 2 doses at 0 and 6 months
- If second dose given before 6 months, give third dose at least 4 months after second dose
MenB-4C (Bexsero) 1
- All indications: 2 doses at 0 and ≥1 month apart
Critical Pitfall to Avoid
