Ziana (Clindamycin 1.2% + Tretinoin 0.025%) for Acne Treatment
For patients aged 12 years or older with acne vulgaris, apply a pea-sized amount of Ziana gel to the entire face once daily at bedtime, as this combination formulation provides superior efficacy compared to either component alone while maintaining excellent tolerability. 1
Indication and Patient Selection
- Approved for topical treatment of acne vulgaris in patients ≥12 years of age 1
- Most effective for moderate comedonal acne and mild-to-moderate papulopustular acne, which represents over 50% of acne patients 2
- Can be combined with systemic therapies (including isotretinoin) for more severe nodulocystic acne 2
- Safety and effectiveness have not been established in pediatric patients <12 years of age 1
Dosing and Application Technique
- Apply a pea-sized amount to the entire face once daily at bedtime 1
- Keep away from eyes, mouth, nasal creases, and mucous membranes 3
- Apply after washing face gently with nonmedicated soap 3
- Wait 20-30 minutes after application before applying any additional products to maximize efficacy 4
Efficacy Data
- In clinical trials involving over 4,500 patients, 21-41% of patients achieved successful treatment using the Evaluator's Global Severity score 1
- The combination significantly reduced lesion counts and improved overall appearance to a greater extent than individual components 5
- Demonstrates rapid onset of action, with benefits visible as early as Week 2 6
- The combination enhances comedolytic efficacy compared to tretinoin alone in moderate-to-severe facial acne 2
Absolute Contraindications
- Regional enteritis, ulcerative colitis, or history of antibiotic-associated colitis (including pseudomembranous colitis) 1
- Known hypersensitivity to clindamycin, tretinoin, lincomycin, or any formulation component 1
Common Adverse Effects
- Erythema, scaling, itching, burning, stinging (local cutaneous reactions) 1
- Dry skin, peeling 1
- Nasopharyngitis, pharyngolaryngeal pain, cough, sinusitis (systemic effects) 1
- Discontinuation rates due to adverse events are ≤1%, indicating excellent tolerability 5, 2
Critical Drug Interactions and Precautions
- Should not be used in combination with erythromycin-containing products 1
- Should not be used with neuromuscular blocking agents due to clindamycin's neuromuscular blocking properties 1
- Concomitant use of topical medications with strong drying effects can increase skin irritation 1
- Avoid keratolytic agents and photosensitizing agents due to tretinoin component 3
- Do not apply with benzoyl peroxide simultaneously to avoid oxidation and inactivation of tretinoin 3
Sun Protection Requirements
- Avoid exposure to sunlight, sunlamps, and tanning beds 1, 3
- Use effective sunscreen and protective clothing when outdoors 3
- Weather extremes (wind, cold) may cause additional irritation 1
Pregnancy and Lactation
- Pregnancy Category C - pregnancy prevention is mandatory for persons of childbearing potential 1, 3
- It is not known whether clindamycin or tretinoin is excreted in human milk after topical application 1
- The American Academy of Dermatology suggests topical tretinoin is likely safe during breastfeeding due to minimal systemic absorption 7
- If breastfeeding, apply after nursing sessions, wash hands thoroughly after application, and avoid application on chest/neck areas that contact the infant 7
Multimodal Therapy Considerations
- The American Academy of Dermatology recommends multimodal therapy combining multiple mechanisms of action to optimize efficacy and reduce antibiotic resistance 1, 3
- When systemic antibiotics are prescribed, they should be used concomitantly with benzoyl peroxide and other topical therapy 3
- One study showed that adding tretinoin + clindamycin to clindamycin/benzoyl peroxide provided no additional clinical advantage 6
Maintenance Strategy
- After initial daily therapy establishes control, maintenance can be achieved with less frequent application (once weekly to three times weekly) 3
- This approach helps sustain visible improvements while minimizing irritation 3
