Needle Size for Reconstitution
For reconstituting medications, use an 18- or 20-gauge needle to efficiently draw up solutions from vials, then switch to the appropriate smaller gauge needle for the actual injection. 1
Reconstitution Needle Specifications
- 18-gauge or 20-gauge needles are specifically recommended for drawing blood or medications from vials during preparation procedures 1
- These larger bore needles allow rapid withdrawal of viscous solutions and facilitate efficient reconstitution without creating excessive negative pressure in the vial 1
- The wider diameter prevents the needle from clogging when drawing up particulate matter or thick suspensions 1
Two-Needle Technique
Always use separate needles for reconstitution versus injection to maintain sterility and needle sharpness:
- Use the 18-20 gauge needle to draw up the reconstituted medication from the vial 1
- Switch to the appropriate smaller gauge needle (based on injection route) before administering to the patient 1
- This prevents dulling of the injection needle, which would increase patient pain 1
Injection Needle Selection After Reconstitution
Once reconstituted, select your injection needle based on the route:
For Intramuscular Injections:
- 22-gauge needles are recommended for viscous oil-based medications (like progesterone in oil) 2

For Subcutaneous Injections:
