Evaluate for Alternative Diagnoses
Light yellow liquid diarrhea suggests:
- Bile acid malabsorption (yellow color indicates bile)
- Small intestinal bacterial overgrowth (SIBO)
- Infectious gastroenteritis
- Malabsorption syndromes (celiac disease, pancreatic insufficiency)
- Irritable bowel syndrome with diarrhea (IBS-D)
- Medication side effects (including PPIs used for GERD, which can cause diarrhea and increase C. difficile risk) 4
Critical Pitfall to Avoid
Do not attribute lower GI symptoms to GERD simply because you have a GERD diagnosis. The 2023 AGA guidelines emphasize that many conditions thought to be GERD-related are actually multifactorial, and causation must be established through objective testing rather than assumption. 1
Recommended Action
Seek evaluation specifically for your diarrhea as a separate problem from your GERD. This requires:
- Stool studies (culture, ova and parasites, C. difficile if recent antibiotics or PPI use)
- Complete blood count and metabolic panel
- Celiac serologies if chronic
- Consider colonoscopy if symptoms persist or alarm features present (blood, weight loss, fever)
Your GERD and diarrhea may coexist as separate conditions, but one does not cause the other.
