Tablet Specifications
2.5 mg tablets: Yellow, round, film-coated biconvex tablets debossed with "C31" on one side and plain on the other side 1
5 mg tablets: Pink, oval, film-coated biconvex tablets debossed with "C32" on one side and plain on the other side 1
Administration Options for Patients Unable to Swallow Whole Tablets
While the standard dosage form is an intact tablet, the FDA label provides alternative administration methods for patients with swallowing difficulties:
Crushed and suspended: Both 2.5 mg and 5 mg tablets may be crushed and suspended in water, 5% dextrose in water (D5W), or apple juice 1
Mixed with applesauce: Crushed tablets can be mixed with applesauce and promptly administered orally 1
Nasogastric tube administration: Tablets may be crushed and suspended in 60 mL of water or D5W and promptly delivered through a nasogastric tube 1
Stability of crushed tablets: Crushed apixaban tablets are stable in water, D5W, apple juice, and applesauce for up to 4 hours 1
Dosing Frequency
Apixaban is administered twice daily regardless of the strength, with doses taken approximately 12 hours apart 2, 1

Common Pitfall to Avoid
- No liquid or injectable formulations exist: Apixaban is only available as an oral tablet formulation 1. For patients requiring parenteral anticoagulation, alternative agents must be used.
