Stieprox (Ciclopirox) Shampoo for Scalp Fungal Infections
Primary Indication and Efficacy
Stieprox (ciclopirox 1-1.5%) shampoo is highly effective for treating seborrheic dermatitis of the scalp, not tinea capitis (scalp ringworm), which requires oral antifungal therapy. 1, 2
- Ciclopirox shampoo demonstrates superior efficacy for seborrheic dermatitis with response rates of 57.9% when used twice weekly and 45.4% when used once weekly, compared to 31.6% for vehicle control 1
- The shampoo contains broad-spectrum antifungal activity against Malassezia yeasts (the causative organism in seborrheic dermatitis) plus anti-inflammatory properties 2, 3
Treatment Protocol for Seborrheic Dermatitis
Acute Treatment Phase (4 weeks)
- Apply ciclopirox 1% shampoo twice weekly for 4 weeks for optimal response rates 1
- Once weekly application is acceptable but produces lower response rates (45.4% vs 57.9%) 1
- Leave shampoo on scalp for 3-5 minutes before rinsing for adequate contact time 1
Maintenance/Prophylaxis Phase (after initial response)
- Continue ciclopirox shampoo once weekly to prevent relapse with only 14.7% relapse rate 1
- Shampooing every 2 weeks is less effective (22.1% relapse rate) but still superior to no prophylaxis (35.5% relapse) 1
- Maintenance therapy should continue long-term given the chronic nature of seborrheic dermatitis 2
Critical Distinction: NOT for Tinea Capitis
Ciclopirox shampoo alone is inadequate for tinea capitis (scalp ringworm), which requires systemic oral antifungal therapy. 4, 5
- Topical therapy alone is not recommended for tinea capitis management 4, 5
- For Trichophyton species: oral terbinafine 250 mg daily (adults >40 kg) for 2-4 weeks is first-line 5
- For Microsporum species: oral griseofulvin 1 g daily (adults >50 kg) for 6-8 weeks is first-line 5
- Antifungal shampoos (ketoconazole 2%, selenium sulfide 1%, or povidone-iodine) serve only as adjunctive therapy to reduce spore transmission in tinea capitis, not as primary treatment 4, 5
Comparative Effectiveness
- Ciclopirox 1.5% shampoo demonstrates at least equivalent efficacy to ketoconazole 2% shampoo for seborrheic dermatitis 6, 7
- Patients rate overall improvement as superior with ciclopirox compared to ketoconazole shampoo (p<0.05) 7
- Ciclopirox shows particular advantage in reducing itching symptoms compared to ketoconazole 6
Safety Profile
- Adverse events are minimal and evenly distributed compared to vehicle control 1
- Local tolerance rated as "good" in >85% of patients 1
- Cosmetic acceptability is excellent with no significant hair or scalp irritation 1, 7
Common Pitfall to Avoid
Do not use ciclopirox shampoo as monotherapy for suspected tinea capitis. If scalp infection presents with kerion, alopecia, lymphadenopathy, or broken hairs, this indicates tinea capitis requiring immediate oral antifungal therapy while awaiting mycological confirmation 4, 5. Delaying systemic treatment risks permanent scarring alopecia 4.
