From the Guidelines

For ear wax resistant to Debrox, I strongly recommend using ear drops containing hydrogen peroxide as an alternative, due to its effectiveness in dissolving the wax through a different chemical mechanism. According to the study by 1, water-based agents, including hydrogen peroxide, have a cerumenolytic effect by inducing hydration and subsequent fragmentation of corneocytes. The evidence suggests that no specific agent is superior to another, and none are superior to either saline or water, but hydrogen peroxide is a viable option [ 1 ].

Some key points to consider when using ear drops for cerumen impaction:

  • Apply 2-3 drops in the affected ear twice daily for 3-5 days to soften the wax
  • Gently irrigate the ear with warm water using a bulb syringe after softening the wax
  • If the impaction persists, consider prescription-strength drops containing sodium bicarbonate or docusate sodium
  • Never insert cotton swabs or other objects into the ear canal as this can push wax deeper or damage the ear canal or eardrum
  • If you experience ear pain, discharge, hearing loss, dizziness, or if the impaction persists after home treatment, seek medical attention promptly

It's essential to note that the evidence profile for therapeutic options has an aggregate evidence quality of Grade B and C, with randomized controlled trials with limitations and cohort studies [ 1 ]. However, the benefits of using effective therapies, such as hydrogen peroxide ear drops, outweigh the potential harm, and the cost is minimal [ 1 ]. Ultimately, the decision to use a particular therapy should be based on the patient's preferences and the healthcare provider's professional judgment.

From the Research

Ear Drops for Resistant Ear Wax

  • There is limited evidence on ear drops specifically for resistant ear wax, including Debrox 2.
  • A study from 2018 found that active treatment with ear drops may result in a greater likelihood of complete wax clearance than no treatment at all, but the quality of evidence was low 2.
  • The same study found no evidence to show that one type of active treatment is more effective than another, and there was no evidence of a difference in efficacy between oil-based and water-based active treatments 2.

Alternative Treatments

  • There is no evidence to show that using saline or water alone is better or worse than commercially produced cerumenolytics 2.
  • Equally, there is also no evidence to show that using saline or water alone is better than no treatment 2.
  • Other studies have investigated the effects of various compounds on reactive oxygen species, but these are not directly relevant to ear drops for resistant ear wax 3, 4, 5, 6.

Adverse Effects

  • Adverse effects such as discomfort, irritation, or pain were reported in some studies, but the evidence was limited and of low quality 2.
  • There were no significant differences in adverse effects between the types of ear drops in the studies that reported this outcome 2.
