For a possible herpes simplex virus (HSV) skin lesion on the mons pubis, I recommend starting oral antiviral therapy promptly with valacyclovir 1g twice daily for 7-10 days, as it is often preferred due to its convenient dosing schedule and good bioavailability 1. First-line treatment options also include acyclovir 400mg three times daily for 7-10 days, or famciclovir 250mg three times daily for 7-10 days. For pain management, acetaminophen or ibuprofen can be used as needed. Some key points to consider in management include:
- Starting treatment as soon as possible, ideally within 72 hours of symptom onset, as antivirals work by inhibiting viral replication and are most effective early in the outbreak.
- Considering laboratory confirmation through PCR testing or viral culture of the lesion if this is a first episode.
- For recurrent episodes, suppressive therapy may be appropriate if outbreaks occur frequently (more than 6 times per year), using valacyclovir 500mg daily or acyclovir 400mg twice daily 1.
- During outbreaks, patients should keep the area clean and dry, avoid tight clothing, and abstain from sexual contact until lesions have completely healed to prevent transmission. Topical treatments like docosanol 10% cream may provide symptomatic relief but are less effective than oral antivirals 1.

The median time to healing among patients with non-aborted lesions (progressing beyond the papule stage) was 4.4 days in the famciclovir 1500 mg single-dose group (n=152) as compared to 6.2 days in the placebo group (n=168). The median time to loss of pain and tenderness was 1.7 days in famciclovir 1500 mg single dose-treated patients vs. 2.9 days in placebo-treated patients. A randomized, double-blind, placebo-controlled trial was conducted in 329 immunocompetent adults with recurrent genital herpes. Patients self-initiated therapy within 6 hours of the first sign or symptom of a recurrent genital herpes episode with either famciclovir 1000 mg twice daily (n=163) or placebo (n=166) for 1 day Meds to order for possible HSV skin lesion on mons pubis:
