Klonopin (clonazepam) is primarily used to treat seizure disorders, panic disorder, and is also conditionally recommended for the treatment of REM sleep behavior disorder (RBD), particularly in cases of isolated RBD or secondary RBD due to a medical condition, as stated in the 2023 American Academy of Sleep Medicine clinical practice guideline 1.

- Seizure disorders: Klonopin enhances the effects of gamma-aminobutyric acid (GABA), reducing abnormal electrical activity in the brain that can cause seizures.
- Panic disorder: Treatment usually starts at 0.25 mg twice daily, potentially increasing to 1-4 mg daily.
- REM sleep behavior disorder (RBD): Klonopin is conditionally recommended for the treatment of RBD, with dosing typically starting at 0.25-1.0 mg administered at bedtime.
Important Considerations
- Klonopin is a controlled substance with potential for dependence and should be taken exactly as prescribed.
- Patients should be aware that abrupt discontinuation can cause withdrawal symptoms.

Conditional Recommendations for RBD Treatment
- The 2023 American Academy of Sleep Medicine clinical practice guideline conditionally recommends the use of clonazepam, immediate-release melatonin, pramipexole, and rivastigmine for the treatment of isolated RBD 1.
- For secondary RBD, the guideline conditionally recommends the use of clonazepam, immediate-release melatonin, and rivastigmine, considering the patient's underlying disease and attendant symptoms 1.
