From the Guidelines

The most effective topical antifungal for vulvovaginal candidiasis is clotrimazole, as it achieves equivalent results to oral antifungal formulations and is effective against most Candida species, including C. albicans [ 1 ]. When considering treatment options, it's essential to classify the infection as either uncomplicated or complicated, with the latter requiring longer treatment durations and potentially different agents [ 1 ]. For uncomplicated vulvovaginal candidiasis, topical agents such as clotrimazole can be used for 3-7 days, while complicated cases may require longer treatment courses or alternative agents like boric acid or flucytosine [ 1 ]. Key factors to consider when selecting a topical antifungal include the specific Candida species involved, the severity of symptoms, and the patient's overall health status, including any potential immunocompromised conditions [ 1 ]. In general, topical antifungals like clotrimazole work by disrupting fungal cell membrane formation, and their effectiveness can be enhanced by maintaining good hygiene practices and avoiding the sharing of personal items that could spread the infection [ 1 ]. It's also important to note that while other studies may provide additional guidance on treating opportunistic infections, the most recent and highest-quality evidence supports the use of clotrimazole as a first-line treatment option for vulvovaginal candidiasis [ 1 ].

From the Research

Topical Antifungals

  • Terbinafine is an effective topical antifungal agent, achieving mycological cure in > 80% of patients with tinea pedis, tinea corporis/cruris, cutaneous candidiasis, and pityriasis versicolor when applied once or twice daily for up to 2 weeks 4
  • Topical terbinafine 1% formulations are at least as effective as miconazole 2% cream and naftifine 1% gel in tinea pedis, and more effective than clotrimazole 1% cream, bifonazole 1% cream, and oxiconazole 1% lotion 4

Comparison with Other Antifungals

  • Terbinafine has shown greater efficacy than griseofulvin in dermatophyte onychomycosis, tinea pedis, and tinea corporis/cruris, and comparable efficacy in children with tinea capitis 4
  • Oral terbinafine is more effective than continuous or intermittent itraconazole in dermatophyte onychomycosis, and is as effective as itraconazole 400 mg/day in tinea pedis 4
  • Combination therapy of terbinafine and itraconazole has a higher clinical and mycological cure rate when compared to the use of either drug alone as monotherapy 5, 6

Efficacy of Combination Therapy

  • A meta-analysis of randomized controlled trials found that combination therapy of itraconazole and terbinafine has a better cure rate when compared to terbinafine monotherapy (RR=2.01 (1.37,2.94); p=0.0003; I2=67%) 6
  • The combination of terbinafine and itraconazole also had a lower recurrence rate compared to itraconazole monotherapy (RR=0.08 (0.02,0.44); p=0.003; I2=0%) 6
