Vaxelis Primary Series Dosing
Vaxelis (DTaP-IPV-Hib-HepB) is administered as a 3-dose primary series at 2,4, and 6 months of age when starting at 2 months old. 1, 2
Standard Dosing Schedule
- The primary vaccination series consists of exactly 3 doses given at ages 2,4, and 6 months, as recommended by the FDA and ACIP 1, 2
- The minimum age for the first dose is 6 weeks, but the standard schedule begins at 2 months 1, 2
- A minimum interval of 4 weeks must occur between each dose 1, 2
Critical Timing Requirements
- Dose 1: Given at 2 months (minimum age 6 weeks) 1, 2
- Dose 2: Given at 4 months (minimum age 10 weeks, minimum 4-week interval from dose 1) 1, 2
- Dose 3: Given at 6 months (minimum age 24 weeks, minimum 4-week interval from dose 2) 1, 2
Essential Hepatitis B Consideration
- Vaxelis is NOT licensed for the birth dose of hepatitis B vaccine - infants must receive a separate monovalent hepatitis B vaccine at birth before starting the Vaxelis series 1, 2
- If the third dose is inadvertently given before 24 weeks of age, an additional dose of hepatitis B vaccine must be administered at age ≥24 weeks to ensure adequate immune response 1, 2
Important Limitations
- Vaxelis should NOT be used for booster doses - separate vaccines are required for the fourth DTaP dose at 15-18 months, the Hib booster at 12-15 months, and the fourth IPV dose at 4-6 years 2
- The maximum age for any Vaxelis dose is 4 years, 364 days (do not administer on or after the fifth birthday) 1
Special Population Note
- For American Indian/Alaska Native infants, both Vaxelis (3-dose series at 2,4,6 months) and monovalent PRP-OMP (2-dose series at 2,4 months) are now preferred options due to early protection against Hib disease 3
