Differentiating Cellulitis from Deep Vein Thrombosis in Unilateral Leg Presentations
In an adult with unilateral leg erythema, warmth, tenderness, and swelling, you must systematically rule out deep vein thrombosis before diagnosing cellulitis, because approximately 12% of patients presenting with apparent lower leg cellulitis have concomitant DVT, and the two conditions share overlapping clinical features that make differentiation challenging. 1
Clinical Features That Help Distinguish the Two Conditions
Features More Suggestive of Cellulitis
- Erythema extending well beyond the area of maximum swelling or tenderness strongly favors cellulitis over DVT 2, 3
- Visible portal of entry (recent trauma, insect bite, tinea pedis with fissuring, chronic venous ulcer) points toward cellulitis 4, 2
