Causes of Dermatographism
Dermatographism is primarily caused by mechanical pressure or shearing forces on the skin that trigger mast cell degranulation and histamine release, though the exact pathophysiology remains incompletely understood. 1, 2
Primary Mechanism
The fundamental cause involves a mechano-immunologic trigger where physical stimulation of the skin leads to mast cell activation, releasing histamine and pro-inflammatory mediators that produce the characteristic linear wheal along the stressed area. 1, 2 This represents an exaggerated cutaneous response to pressure that occurs at a lower threshold than in normal individuals. 3
Idiopathic vs. Secondary Causes
Idiopathic (Most Common)
- The majority of dermatographism cases have no identifiable underlying cause and are classified as a physical urticaria subtype. 4, 1
- Simple dermographism occurs in 1.5-5% of healthy individuals and is considered a normal physiological phenomenon. 5
Secondary Triggers and Associated Conditions
Psychogenic factors emerge as the most common identifiable trigger, initiating symptoms in 30% of patients with symptomatic dermographism. 5
Drug-induced reactions account for 12.5% of cases, with symptoms beginning following drug-induced urticarial rash. 5, 2
Infectious agents can trigger dermographism:
- Scabies infection has a documented temporal relationship with onset 5
- Post-streptococcal glomerulonephritis has been associated with development 5
Atopic conditions are present in 12.5% of symptomatic dermographism patients, particularly those with house dust mite sensitivity. 5
Endocrine disorders show association:
Mastocytosis can present with prominent dermatographism, particularly in pediatric cutaneous mastocytosis where erythema, swelling, and blister formation after stroking occurs with associated pruritus. 4
Important Clinical Distinctions
The British Journal of Dermatology classifies symptomatic dermographism as a mechanical physical urticaria, distinct from ordinary spontaneous urticaria. 4 Unlike spontaneous urticaria, dermographism is reproducibly induced by the same physical stimulus rather than occurring spontaneously. 4
Common Pitfalls
Do not assume all dermographism is benign. While simple dermographism is physiologic, symptomatic dermographism significantly impacts quality of life and warrants investigation for underlying triggers. 5, 1
Do not overlook temporal relationships. A close temporal association between symptom onset and psychogenic stress, drug exposure, or scabies infection should prompt consideration of these as causative factors. 5
Do not miss systemic mastocytosis. When dermographism is accompanied by flushing, systemic symptoms, or occurs in children with pigmented lesions, evaluate for mastocytosis with Darier's sign testing. 4
Recognize genetic predisposition. While rare, familial cases exist, suggesting hereditary factors in some patients. 4, 2
