Important Timing Considerations with Supplements
While food is not required, specific timing rules apply when taking supplements or antacids containing polyvalent cations (aluminum, magnesium, iron, or calcium):
If taking antacids containing aluminum or magnesium, take Biktarvy at least 2 hours before or 6 hours after these antacids. 3
If taking supplements or antacids containing iron or calcium, take Biktarvy WITH FOOD at the same time as these supplements. 3
These cations can reduce bictegravir absorption through chelation, potentially leading to subtherapeutic drug levels and treatment failure. 2
Special Populations
For children unable to swallow the whole tablet, it can be split and each part taken separately within approximately 10 minutes—food is still not required. 3
For patients on dialysis, take the daily dose following dialysis, with or without food. 3
Pregnant patients taking supplements containing aluminum, magnesium, iron, or calcium should discuss timing strategies with their healthcare provider. 3
Common Pitfalls to Avoid
Do not confuse Biktarvy's food-independent dosing with elvitegravir-based regimens, which must be taken with food for adequate absorption. 1, 2
Do not ignore the timing requirements for polyvalent cation supplements—this is the most common drug interaction that can compromise efficacy. 2, 3
Do not crush Biktarvy tablets in food (such as apple compote), as this significantly reduces emtricitabine and tenofovir alafenamide exposure by approximately 30% and may lead to virologic failure. 4 If the tablet cannot be swallowed whole, dissolving it in water maintains bioequivalence. 4
