Understanding BI-RADS 2 Classification

BI-RADS Category 2 represents definitively benign findings with essentially zero malignancy risk. 1, 2 These findings include:

  • Intramammary lymph nodes
  • Vascular calcifications
  • Implants
  • Architectural distortion clearly related to prior surgery 1

A large retrospective study of 192 patients with BI-RADS 2 lesions confirmed 0% malignancy rate with follow-up ranging 2-9 years, demonstrating that breast biopsies are unnecessary in these cases. 2

Management Algorithm

Common Pitfall to Avoid

Do not confuse BI-RADS 2 with BI-RADS 0 (incomplete assessment). BI-RADS 0 requires additional imaging evaluation before any management decision can be made, whereas BI-RADS 2 is a complete and definitive benign assessment. 1
