Evidence-Based Management for Adult Acute Diarrhea
The most recent guidelines for adult self-medication of acute diarrhea do not include zinc supplementation as a recommended intervention. 1 The evidence and recommendations for zinc are specific to pediatric populations, particularly children aged 6 months to 5 years in zinc-deficient regions or with malnutrition. 3, 4, 2
Recommended Treatment Approach for Adults
First-line management should focus on:
- Loperamide 2 mg as the anti-diarrheal drug of choice, with flexible dosing according to loose bowel movements 1
- Adequate fluid intake guided by thirst, using glucose-containing drinks (lemonades, sweet sodas, fruit juices) or electrolyte-rich soups 1
- Small, light meals guided by appetite, avoiding fatty, heavy, spicy foods and caffeine 1
Why Zinc Is Not Indicated for Adults
The clinical trial evidence and guideline recommendations for zinc supplementation are exclusively derived from pediatric studies. 3, 4, 2, 5, 6, 7, 8 The 2001 European guidelines for adult acute diarrhea management, which represent the standard of care, make no mention of zinc supplementation as a therapeutic option. 1
When to Consider Alternative Interventions
Probiotics may be offered to reduce symptom severity and duration in immunocompetent adults with infectious diarrhea, though this carries only a weak recommendation with moderate-quality evidence. 2
Antimicrobials (quinolones as first choice, cotrimoxazole as second) are reserved for:
- Moderate to severe traveler's diarrhea 1
- Diarrhea with fever and/or bloody stools 1
- Empirical self-medication during travel abroad based on prior medical advice 1
Red Flags Requiring Medical Evaluation
Seek medical attention if: 1
- No improvement within 48 hours 1
- High fever beyond 38.5°C with frank blood in stools (dysentery) 1
- Severe vomiting leading to rapid dehydration 1
- Signs of obvious dehydration 1
- Abdominal distension 1
Common Pitfall to Avoid
Do not extrapolate pediatric zinc supplementation recommendations to adults. The benefit of zinc in reducing diarrhea duration by 10-27 hours has only been demonstrated in malnourished children from zinc-deficient populations. 3, 2, 8 There is no evidence that zinc supplementation benefits well-nourished adults in developed countries. 2
