According to the study published in 2023 1, pregabalin has been shown to be effective in treating pain in diabetic peripheral neuropathy, with eight high-quality studies and seven medium-quality studies supporting its role. The study also suggests that gabapentinoids, including pregabalin, can be considered as a first-line treatment for neuropathic pain.
The advantages and disadvantages of each medication are as follows:
- Gabapentin:
- Advantages: extensive clinical experience, affordability
- Disadvantages: three-times-daily dosing, potential for dizziness, somnolence, and peripheral edema
- Pregabalin:
- Advantages: more predictable absorption, twice-daily dosing, faster onset of action
- Disadvantages: more expensive, may cause weight gain, dizziness, and somnolence
- Duloxetine:
- Advantages: once-daily dosing, added benefit of treating comorbid depression and anxiety
- Disadvantages: may cause nausea, dry mouth, constipation, and has potential for discontinuation syndrome if stopped abruptly
The treatment choice should consider the patient's pain characteristics, comorbidities, medication schedule preferences, and cost considerations, and starting with lower doses and titrating slowly can minimize side effects with all three medications.
