How to Use Protopic (Tacrolimus) Ointment
Apply a thin layer of Protopic ointment twice daily to affected areas, using the minimum amount needed to control symptoms, and discontinue when signs and symptoms resolve. 1
Strength Selection by Age
- Adults and children ≥16 years: Use either 0.03% or 0.1% tacrolimus ointment 1
- Children 2-15 years: Use only 0.03% tacrolimus ointment 1
- Children <2 years: Protopic is contraindicated in this age group 2, 1
Application Instructions
Frequency and technique:
- Apply twice daily to affected skin areas only 1
- Use a thin layer and rub in gently and completely 1
- Apply the minimum amount necessary to control atopic dermatitis signs and symptoms 1
- Limit application strictly to areas involved with atopic dermatitis 1
Hand hygiene:
- Wash hands before applying 1
- If you are a caregiver or not treating your hands, wash hands with soap and water after application to remove residual ointment 1
Timing considerations:
- Do not bathe, shower, or swim immediately after applying, as this could wash off the ointment 1
- If using moisturizers, apply them after Protopic ointment 1
Duration and Discontinuation
When to stop:
- Discontinue when signs and symptoms (itch, rash, redness) resolve 1
- Avoid continuous long-term use; use short-term or intermittent long-term application instead 2, 1
- Treatment may be repeated with breaks in between as needed 1
When to reassess:
- If symptoms do not improve within 6 weeks, patients should be re-examined to confirm the diagnosis of atopic dermatitis 1
- Clinical improvements typically occur within the first week of treatment 3
Critical Safety Precautions
What to avoid:
- Do not use occlusive dressings or bandages over treated areas (normal clothing is acceptable) 1
- Do not use on immunocompromised patients 2
- Minimize or avoid natural or artificial sunlight exposure during treatment, even when ointment is not on the skin 1
- Do not use ultraviolet light therapy, sun lamps, or tanning beds during treatment 1
- If sun exposure is necessary, wear loose-fitting protective clothing over treated areas 1
- Avoid getting ointment in eyes or mouth; do not swallow 1
Pre-treatment requirements:
- Treat any active skin infections before initiating tacrolimus therapy 2, 1
- The safety and efficacy in clinically infected atopic dermatitis have not been established 1
Special Considerations
Sensitive areas:
- Tacrolimus is particularly well-suited for face and skin-fold regions where corticosteroid-induced skin atrophy is a concern 4
- Head and neck involvement shows comparable efficacy and safety to non-head/neck areas 5
- For eyelid dermatitis, 0.1% tacrolimus has demonstrated safety and efficacy in clinical trials 6
Common initial side effects:
- Transient skin burning and itching at application sites are the most common adverse events 3, 7
- These symptoms typically decrease after the first few days of treatment 3, 5
- The prevalence of application site adverse events decreases rapidly during initial treatment days 5
