NICU Admission Criteria for Neonatal Jaundice
Aapke newborn ko NICU mein tab admit karna chahiye jab Total Serum Bilirubin (TSB) level exchange transfusion threshold ke barabar ya usse 0-2 mg/dL neeche ho - isko "escalation of care" level kehte hain - aur agar intensive phototherapy ke baad bhi TSB badhta rahe, to turant neonatologist se consult karke NICU transfer karna zaroori hai. 1
NICU Transfer ke Specific Criteria
Primary Indication: Escalation of Care Level
Jab bhi TSB level exchange transfusion threshold tak pahunch jaye ya usse 0-2 mg/dL neeche ho, to yeh critical situation hai. Is stage par:
- Turant IV hydration shuru karein
- Emergency intensive phototherapy dein
- Har 2 ghante mein TSB measure karein 1
Agar in interventions ke baad bhi TSB level badhta rahe, to neonatologist se consult karke NICU transfer arrange karein 1. Yeh decision kernicterus (brain damage) se bachane ke liye critical hai.
Risk Factors jo NICU Admission ki Zaroorat Badha Dete Hain
Kuch specific situations mein NICU care ki zaroorat jaldi pad sakti hai:
- Rapid TSB rise: Pehle 24 hours mein ≥0.3 mg/dL per hour ya uske baad ≥0.2 mg/dL per hour - yeh hemolysis ka sign hai 1
- Intensive phototherapy ke baad bhi TSB ka badhna 1
- Gestational age <38 weeks 1
- Positive Direct Antiglobulin Test (DAT) ya hemolytic disease ka suspicion 1

Clinical Warning Signs
Agar baby mein yeh neurological signs dikhen, to turant NICU transfer karein:
- Feeding pattern mein deterioration
- Lethargy (excessive sleepiness)
- Inconsolability (excessive crying)
- High-pitched crying
- Hypotonia (floppy baby) ya hypertonia (stiff baby)
- Opisthotonus (arching of back)
- Retrocollis (head tilting back)
- Fever 1
Home Phototherapy vs Hospital Admission
Important caveat: Home phototherapy sirf un babies ke liye option hai jo specific criteria meet karte hain aur jinki TSB level phototherapy threshold se thodi upar hai - escalation of care level par nahi 1. Home phototherapy tab consider kar sakte hain jab:
- Baby already discharge ho chuka ho
- TSB phototherapy threshold se upar ho lekin escalation level tak nahi
- Koi serious risk factors na hon
- Proper follow-up available ho
Monitoring Protocol NICU Mein
NICU admission ke baad:
- TSB measurement har 2 ghante mein jab tak escalation period khatam na ho 1
- Continuous intensive phototherapy
- IV hydration maintain karein
- Underlying cause ka evaluation (G6PD testing, blood group incompatibility, etc.) 1
Common Pitfall to Avoid
Transcutaneous bilirubin (TcB) readings par treatment decisions mat lein - sirf TSB (blood test) use karein treatment decisions ke liye 1. TcB screening ke liye theek hai, lekin jab TSB phototherapy threshold ke 3.0 mg/dL ke andar ho ya ≥15 mg/dL ho, to TSB measure karna mandatory hai 1.
