Benidipine Daily Dose
The standard daily dose of benidipine for hypertension is 4-8 mg once daily, starting at 2-4 mg and titrating up to 8 mg as needed to achieve blood pressure control. 1
Dosing Algorithm
Initial dose: Start with 2-4 mg once daily 1, 2
Titration schedule:
- If target blood pressure is not achieved within 2 weeks, increase to 8 mg once daily 3, 2
- The maximum recommended dose is 8 mg per day 4, 5, 6
- Titration should proceed over 7-14 days to allow full assessment of patient response 1
Target blood pressure: Aim for <140/90 mmHg in patients without comorbidities, or <130/80 mmHg in high-risk patients with cardiovascular disease, diabetes, or chronic kidney disease 7
Clinical Context
While the FDA labeling provided references amlodipine dosing (5-10 mg daily) 1, the research evidence consistently demonstrates that benidipine is effective at lower doses of 4-8 mg daily. This is a T-/L-type calcium channel blocker, distinguishing it from pure L-type blockers like amlodipine.
Key advantages of benidipine at these doses:
- Provides sustained 24-hour blood pressure control 2
- Reduces proteinuria more effectively than L-type calcium channel blockers alone, particularly at the 8 mg dose 4, 5, 6
- Decreases plasma aldosterone concentration, especially at 8 mg daily 4
- Shows particular benefit in elderly patients (≥65 years) with chronic kidney disease 3
Special Populations
Elderly patients: The same 2-8 mg dosing range applies, with studies showing excellent tolerability and potentially greater antiproteinuric effects in patients ≥65 years 3, 2
Patients with chronic kidney disease: Benidipine 4-8 mg daily is particularly beneficial, as it dilates both afferent and efferent glomerular arterioles through T-type calcium channel blockade, reducing intraglomerular pressure and proteinuria 5, 6, 8
Important Caveats
- Benidipine can be combined with RAS inhibitors (ACE inhibitors or ARBs) for additional blood pressure control and renal protection 5, 3, 9
- The drug binds to receptors via a "membrane approach," providing sustained effect independent of blood concentration 8
- Compliance rates are excellent (98.7%) with once-daily dosing 2
- Adverse events occur in approximately 15% of patients, with discontinuation rates around 4% 2
