Child Zyrtec Dosage
For children 6 years and older, give cetirizine 10 mg once daily (or 5 mg for less severe symptoms); for children 2-5 years, give 5 mg (2.5 mg twice daily or 5 mg once daily); for infants 6-23 months, give 0.25 mg/kg twice daily (approximately 2.5 mg total daily dose); children under 6 months should not receive cetirizine. 1
Age-Based Dosing Algorithm
Children ≥6 Years
- Standard dose: 10 mg once daily 1
- Mild symptoms: 5 mg once daily may be sufficient 1
- Maximum: Do not exceed 10 mg in 24 hours 1
- The FDA label explicitly states this is the approved dosing for this age group 1
Children 2-5 Years
- Dose: 5 mg daily (can be given as 2.5 mg twice daily or 5 mg once daily)
- Multiple studies support safety and efficacy in this age range 2, 3
- A study of 47 children aged 2-6 years demonstrated excellent improvement with 5 mg daily for allergic rhinitis 2
Infants 6-23 Months
- Dose: 0.25 mg/kg twice daily (total approximately 4-5 mg/day) 4, 5
- This dosing was validated in the landmark ETAC study of 343 infants, showing comparable drug exposure to adults taking 5 mg once daily 5
- A randomized, double-blind, placebo-controlled safety study specifically in infants 6-11 months confirmed this dose is safe with no CNS or cardiac effects 4
Children <6 Months
- Not recommended - consult a physician 1
- Insufficient safety data in this age group
Important Clinical Considerations
Second-generation antihistamines like cetirizine have an excellent safety profile in young children, which is critical given the FDA's concerns about OTC cough and cold medications in children under 6 years 6. Guidelines specifically note that cetirizine, when used in young children, has been "shown to be well tolerated and to have a very good safety profile" 6.
Special Populations Requiring Dose Adjustment
Renal impairment: Halve the cetirizine dose; avoid entirely if creatinine clearance <10 mL/min 7
Hepatic impairment: Use with caution; consult physician 1
Elderly (≥65 years): Consult physician before use 1
Common Pitfalls to Avoid
- Do not use OTC combination cough/cold products in children under 6 years - these have documented fatalities and lack efficacy 6
- Do not confuse cetirizine with first-generation antihistamines that cause sedation and have poorer safety profiles
- Weight-based dosing is more accurate in infants and toddlers than age-based dosing 5
- The most common adverse events (headache, pharyngitis, abdominal pain) occur at rates similar to placebo 8
Formulation Selection
Use oral drops or syrup for children under 6 years for accurate dosing. The 10 mg tablet formulation is appropriate for children ≥6 years 1.
