Management of Swelling After Nail Phenolisation
Swelling after nail phenolisation should be managed with daily soaking in betadine or salt water solution, followed by application of topical antimicrobial/anti-inflammatory agents (such as cortisporin otic solution), and dry sterile dressing changes until healing is complete.
Expected Post-Procedure Course
Swelling and inflammation are normal sequelae of phenol matricectomy. Understanding the typical healing timeline helps distinguish normal post-operative changes from complications:
- Pain duration: Expect 7-16 days of discomfort 1, 2
- Drainage/oozing: Typically continues for 15-18 days 1, 2
- Tissue normalization: Usually occurs within 7-16 days 2
- Complete healing: Generally achieved within 33-34 days 3
Specific Management Protocol
Immediate Post-Operative Period (Day 1)
- Remove the Adaptic drain along with accumulated coagulum 4
- This removal of necrotic tissue facilitates drainage and reduces inflammatory burden
Ongoing Daily Care (Until Healing)
- Soaking regimen: Betadine or salt water solution 4
- Topical application: Cortisporin otic solution (provides both antimicrobial and anti-inflammatory effects) 4
- Dressing: Dry sterile dressing after each treatment 4
Important Clinical Considerations
No advantage to specialized dressings: Studies comparing medicated dressings (10% povidone iodine), hydrogel dressings, and simple paraffin gauze showed no difference in healing time or infection rates 3. In fact, hydrogel dressings were associated with more complications like hypergranulation 3.
Infection rates are low: Clinical infection occurs in only 2-7% of cases when proper technique is used 3. The presence of drainage and swelling alone does not indicate infection.
Red Flags Requiring Further Evaluation
While swelling is expected, watch for:
- Excessive pain beyond 2-3 weeks
- Purulent discharge with foul odor
- Spreading erythema beyond the immediate surgical site
- Systemic symptoms (fever, malaise)
- Hypergranulation tissue formation
Common Pitfall
Do not confuse normal post-phenolisation drainage with infection. Prolonged drainage (up to 18 days) is a known characteristic of phenol matricectomy and represents tissue necrosis and healing, not necessarily infection 1, 2. The drainage should gradually decrease over 2-3 weeks.
