Vitamin B12 Does Not Cause Red, Dry, or Chapped Lips
Vitamin B12 supplementation is not associated with causing red, dry, or chapped lips. In fact, the opposite is true: vitamin B12 deficiency can cause glossitis (inflammation of the tongue and oral mucosa), which may extend to lip involvement 1.
Understanding the Relationship Between B12 and Lip Symptoms
B12 Deficiency Causes Oral Symptoms, Not Supplementation
The 2024 NICE guidelines clearly identify glossitis as a common symptom of vitamin B12 deficiency, not supplementation 1. When patients have inadequate B12 levels, they may develop:
- Glossitis (inflamed, red tongue)
- Oral mucosal changes
- Potentially associated lip inflammation
These symptoms resolve with B12 treatment, not worsen.
Known Dermatologic Side Effects of B12 Supplementation
The documented cutaneous reactions to vitamin B12 therapy are distinct and do not include chapped lips 2, 3, 4:
Actual B12 supplementation side effects include:
- Acneiform eruptions (most common dermatologic reaction) 3, 4
- Rosacea-like eruptions
- Itching and transitory rash 2
- Allergic site reactions at injection sites
- Rare anaphylaxis (with parenteral administration) 2
A 2025 case report documented a 67-year-old woman who developed facial acne after high-dose B12 for pernicious anemia, but no lip symptoms were reported 4.
Clinical Reasoning
If a patient presents with red, dry, or chapped lips while taking B12:
Consider alternative causes:
- Environmental factors (cold, wind, sun exposure)
- Contact dermatitis (lip products, toothpaste)
- Angular cheilitis (often related to riboflavin/B2 or iron deficiency, not B12)
- Dehydration
- Other nutritional deficiencies (iron, riboflavin, folate)
Evaluate for persistent B12 deficiency: If the patient recently started B12 for documented deficiency, glossitis and oral symptoms may still be present and require time to resolve
Rule out hypersensitivity: While rare, some patients develop allergic reactions to B12 formulations, but these typically manifest as acne, rash, or injection site reactions—not isolated lip symptoms 5
Important Caveat
The evidence base does not support a causal relationship between B12 supplementation and chapped lips. Do not discontinue necessary B12 supplementation based on lip symptoms alone, as untreated B12 deficiency leads to serious neurologic complications including peripheral neuropathy, myelopathy, and cognitive impairment 1.
