Concerta Composition
Concerta (methylphenidate hydrochloride extended-release tablets) contains methylphenidate HCl as the active ingredient in strengths of 18,27,36, or 54 mg, along with multiple inactive ingredients including butylated hydroxytoluene, carnauba wax, cellulose acetate, hypromellose, lactose, phosphoric acid, poloxamer, polyethylene glycol, polyethylene oxides, povidone, propylene glycol, sodium chloride, stearic acid, succinic acid, synthetic iron oxides, titanium dioxide, and triacetin. 1
Active Ingredient
The active pharmaceutical ingredient is methylphenidate hydrochloride (HCl), a racemic mixture of d- and l-isomers, with the d-isomer being more pharmacologically active. 1 The chemical formula is C₁₄H₁₉NO₂·HCl with a molecular weight of 269.77. Methylphenidate HCl appears as a white, odorless crystalline powder that is freely soluble in water and methanol. 1
Inactive Ingredients (Excipients)
The complete list of inert ingredients includes: 1
- Butylated hydroxytoluene (antioxidant)
- Carnauba wax (coating agent)
- Cellulose acetate (semipermeable membrane component)
- Hypromellose (polymer)
- Lactose (filler)
- Phosphoric acid (pH modifier)
- Poloxamer (surfactant)
- Polyethylene glycol (polymer)
- Polyethylene oxides (osmotic agent)
- Povidone (binder)
- Propylene glycol (solvent)
- Sodium chloride (osmotic agent)
- Stearic acid (lubricant)
- Succinic acid (pH modifier)
- Synthetic iron oxides (colorant)
- Titanium dioxide (colorant/opacifier)
- Triacetin (plasticizer)
Unique Delivery System (OROS Technology)
Concerta utilizes a sophisticated osmotic-release oral system (OROS) that distinguishes it from other methylphenidate formulations. 1 The tablet consists of:
- Trilayer osmotic core: Two drug-containing layers plus one osmotically active "push" layer
- Semipermeable membrane: Controls water entry rate
- Immediate-release overcoat: Provides initial methylphenidate dose within 1 hour
- Precision laser-drilled orifice: Allows controlled drug release over 12 hours
The biologically inert tablet shell and core components remain intact during gastrointestinal transit and are eliminated in stool. 1 This is clinically important—patients and families should be counseled that seeing the tablet shell in stool is normal and does not indicate treatment failure.
Critical Caveat About Generic Substitution
Not all generic "Concerta" products use OROS technology. Non-OROS generic methylphenidate ER products have demonstrated statistically significant differences in dissolution profiles and clinical effectiveness compared to brand-name Concerta. 2, 3 In one clinical case series, switching from non-OROS to OROS formulation at identical doses resulted in a 23-point reduction in Inattention Scale T-scores (p<0.0001). 2 Additionally, some generic immediate-release methylphenidate products have been found to contain N-nitroso-methylphenidate (a probable human carcinogen) above regulatory thresholds. 3
When prescribing, specify "brand medically necessary" or ensure the generic uses OROS technology to avoid therapeutic inequivalence.
