Do Not Cut Paxil Extended-Release Tablets
Paxil ER (paroxetine extended-release) tablets should NOT be cut, crushed, or chewed—they must be swallowed whole. 1
Why Cutting Is Contraindicated
The FDA drug label explicitly states that "PAXIL should not be chewed or crushed, and should be swallowed whole" 1. This is a critical pharmaceutical requirement for extended-release formulations.
Consequences of Cutting ER Tablets
When you cut an extended-release tablet, you:
- Destroy the controlled-release mechanism that was specifically engineered to delay drug release until after gastric passage, then provide gradual absorption over 4-5 hours 2
- Risk immediate-release of the full dose, potentially causing:
Clinical Implications
If dose adjustment is needed:
- Use the appropriate commercially available strength (12.5 mg, 25 mg doses are available and effective) 5
- Do not attempt to create custom doses by cutting tablets
- The ER formulation specifically provides improved GI tolerability compared to immediate-release paroxetine 3, 2
Important safety considerations for paroxetine:
- Paroxetine has the highest risk of discontinuation syndrome among SSRIs 4
- It requires careful dose titration and should never be abruptly stopped
- CYP2D6 poor metabolizers may need only 25% of standard dosing 6
If a patient cannot swallow tablets whole, consider switching to immediate-release paroxetine tablets (which can be split if scored) or selecting an alternative SSRI rather than compromising the ER formulation's integrity.
