Tapering Invega (Paliperidone) and Latuda (Lurasidone)
Gradual antipsychotic withdrawal over a period greater than 1 month is recommended to minimize discontinuation effects including dyskinesias, parkinsonian symptoms, dystonias, and neuroleptic malignant syndrome. 1
General Antipsychotic Tapering Principles
Abrupt discontinuation of antipsychotics must be avoided as it is associated with dyskinesias, parkinsonian symptoms, dystonias, neuroleptic malignant syndrome, and markedly increased risk of mood destabilization and withdrawal phenomena. 1, 2
Timing Before Initiating Taper
- Maintain antipsychotic therapy for at least 12 months after achieving remission before considering dose reduction to lower relapse risk. 2
- For first-episode psychosis, continue treatment for 1–2 years after the episode before tapering due to heightened relapse risk. 2
Recommended Tapering Speed
- For patients on chronic antipsychotic therapy (many years), taper at approximately 10% of the current dose per month to minimize withdrawal symptoms and preserve stability. 2
- Withdrawal should extend over a period greater than 1 month in most cases. 1
- Discontinuing over 10–14 days is insufficient and carries higher risk of withdrawal symptoms. 2
Critical Monitoring
- The 2- to 6-week interval when plasma drug levels are declining is high-risk; monitor closely for mood destabilization or breakthrough psychotic symptoms and be prepared to pause the taper. 2
- Re-escalate dosing if persisting withdrawal symptoms cause patient distress. 1
- Allow 1–6 month intervals between medication adjustments in the absence of clinical deterioration. 2
Latuda (Lurasidone) Specific Considerations
Standard Dosing Context
- Latuda does not require initial dose titration when starting or switching. 3
- The recommended starting dose is 40 mg daily for schizophrenia (adults and adolescents 13-17 years) and 20 mg daily for bipolar depression (adults and pediatric patients 10-17 years). 3
- Latuda must be taken with food (at least 350 calories) as administration with food increases absorption approximately 2-fold (AUC) and 3-fold (Cmax). 3
Switching From Latuda
- When switching from lurasidone, three successful strategies have been demonstrated: starting the new antipsychotic at 40 mg/day for 2 weeks, 80 mg/day for 2 weeks, or 40 mg/day for 1 week followed by 80 mg/day the second week. 4
- The pre-switch antipsychotic (Latuda) should be tapered to 50% of the original dose by day 7 and discontinued by the end of week 2 when switching to another antipsychotic. 4
Tapering Latuda for Discontinuation
- Apply the general antipsychotic tapering principles: gradual withdrawal over greater than 1 month, reducing approximately 10% of the current dose per month for chronic use. 1, 2
- Monitor closely during the 2-6 week period when plasma levels are declining. 2
Invega (Paliperidone) Specific Considerations
Oral Paliperidone Extended-Release
- When discontinuing oral paliperidone, follow the general antipsychotic tapering principles: gradual withdrawal over greater than 1 month with approximately 10% monthly dose reductions for chronic use. 1, 2
Paliperidone Palmitate Long-Acting Injectable (LAI)
- For patients on paliperidone palmitate LAI who need to discontinue, the extended-release pharmacokinetic profile means plasma levels decline gradually over months after the last injection. 5, 6
- The re-initiation schedule depends on duration since the last injection: if the last maintenance dose was >6 weeks previously, specific re-initiation protocols apply. 6
- When switching from paliperidone palmitate to oral antipsychotics, the new oral medication can be initiated at the time of what would have been the next scheduled injection. 5, 6
Switching Between Formulations
- When switching from oral paliperidone to paliperidone palmitate, the recommended initiation is 150 mg eq. (234 mg) on day 1 followed by 100 mg eq. (156 mg) on day 8, both deltoid injections, with no oral supplementation required. 5, 6
- When switching from paliperidone palmitate to oral antipsychotics, cross-tapering strategies should be considered to reduce rebound and withdrawal symptoms. 7
Common Pitfalls and Caveats
Avoid these critical errors:
- Never abruptly discontinue either medication as this markedly increases risk of withdrawal phenomena and relapse. 1, 2
- Do not use a 10-14 day taper as this is insufficient and associated with higher withdrawal risk. 2
- Do not forget that Latuda requires food administration; this remains important even during tapering. 3
- Do not overlook renal or hepatic impairment which may require dose adjustments during tapering. 3
Supportive interventions:
- Nonpharmacological psychosocial interventions including redirection techniques, environmental interventions, social engagement, and maintaining sleep-wake cycles can help mitigate behavioral symptoms during antipsychotic reduction. 1
- Patient education about risks and benefits of tapering improves success rates. 1
