Symbicort Dosing
For asthma, adults and adolescents ≥12 years should use 2 inhalations of Symbicort 80/4.5 or 160/4.5 twice daily (morning and evening, approximately 12 hours apart), with a maximum dose of Symbicort 160/4.5 two inhalations twice daily; children 6 to <12 years should use 2 inhalations of Symbicort 80/4.5 twice daily. 1
Asthma Dosing by Age Group
Adults and Adolescents (≥12 Years)
- Starting dose: 2 inhalations of Symbicort 80/4.5 or 160/4.5 twice daily, based on asthma severity and current control 1
- Maximum dose: Symbicort 160/4.5, two inhalations twice daily 1
- Dose adjustment: If inadequate response after 1-2 weeks on Symbicort 80/4.5, consider switching to Symbicort 160/4.5 for additional control 1
- Timing: Administer morning and evening, approximately 12 hours apart 1
Pediatric Patients (6 to <12 Years)
- Dose: 2 inhalations of Symbicort 80/4.5 twice daily 1
- Important limitation: Only the 80/4.5 strength is appropriate for this age group 1
Children <6 Years
- Symbicort is not indicated for children under 6 years of age 1
COPD Dosing
- Recommended dose: Symbicort 160/4.5, two inhalations twice daily 1
- Important note: Symbicort 160/4.5 is the only strength indicated for COPD treatment 1
Administration Technique
- Priming: Prime before first use by releasing 2 test sprays into the air (away from face), shaking well for 5 seconds before each spray 1
- Re-priming: Prime again if inhaler unused for >7 days or if dropped 1
- Post-inhalation: Rinse mouth with water without swallowing after each use 1
- Route: Orally inhaled only 1
Critical Dosing Limitations
- Do not exceed prescribed dose: More frequent administration or higher number of inhalations than 2 inhalations twice daily is not recommended, as higher formoterol doses increase adverse effect risk 1
- No additional LABA: Patients using Symbicort should not use additional long-acting beta-agonists for any reason 1
- Rescue medication: Use a short-acting beta2-agonist (SABA) for acute symptom relief between doses, not additional Symbicort 1
Maintenance and Reliever Therapy (SMART/MART) Approach
For moderate-to-severe persistent asthma (Steps 3-4), the 2020 NAEPP guidelines strongly recommend ICS-formoterol as both daily controller and reliever therapy, which provides superior exacerbation reduction compared to higher-dose ICS alone or same-dose ICS-LABA with SABA rescue. 2, 3
SMART Dosing Regimen
- Maintenance: Budesonide-formoterol 160/4.5 μg, one inhalation once or twice daily (Step 3) or two inhalations twice daily (Step 4) 4
- Reliever: Additional inhalations of budesonide-formoterol 160/4.5 μg, one inhalation as needed for symptom relief 4
- Maximum daily total: Up to 12 inhalations in any single day for adults and adolescents (delivering 54 μg formoterol total) 4
- Evidence base: This approach reduces severe exacerbations at lower overall ICS exposure compared to fixed-dose regimens 4, 5
Important SMART Caveats
- FDA labeling vs. guidelines: The FDA-approved Symbicort labeling does not explicitly include SMART dosing, but the 2020 NAEPP guidelines strongly recommend this approach for Steps 3-4 2, 3, 1
- Formoterol-specific: SMART has only been validated with formoterol-containing ICS combinations, not other LABAs 4
- Patient education essential: Requires careful explanation and preferably a written asthma action plan to ensure proper self-management 4
Onset and Duration of Benefit
- Rapid onset: Improvement in asthma control can occur within 15 minutes of beginning treatment 1
- Maximum benefit: May not be achieved for 2 weeks or longer after starting treatment 1
- Individual variation: Time to onset and degree of symptom relief varies among patients 1
Common Pitfalls to Avoid
- Not using for acute relief: Symbicort is not indicated for relief of acute bronchospasm in either asthma or COPD 1
- Wrong strength for children: Never use Symbicort 160/4.5 in children 6 to <12 years 1
- Wrong strength for COPD: Never use Symbicort 80/4.5 for COPD; only the 160/4.5 strength is indicated 1
- Exceeding maximum dose: Do not exceed 2 inhalations twice daily of the prescribed strength in standard fixed-dose regimens 1
