Maximum Dose of Entresto in Adult CKD Patients
The maximum dose of Entresto (sacubitril/valsartan) for adult patients with chronic kidney disease is 97/103 mg twice daily, which is the same target dose as for patients without CKD. 1
Dosing Algorithm Based on Renal Function
For Severe Renal Impairment (eGFR <30 mL/min/1.73 m²)
- Start at half the usual starting dose: Begin with 24/26 mg twice daily instead of the standard 49/51 mg twice daily 1
- Titrate upward every 2-4 weeks as tolerated, following the standard dose escalation: 24/26 mg → 49/51 mg → 97/103 mg twice daily 1
- The maximum target dose remains 97/103 mg twice daily, even in severe CKD 1
For Mild to Moderate Renal Impairment (eGFR 30-89 mL/min/1.73 m²)
- No starting dose adjustment needed 1
- Begin with standard starting dose of 49/51 mg twice daily 1
- Titrate to maximum dose of 97/103 mg twice daily as tolerated 1
Evidence Supporting Use in Advanced CKD
The UK HARP-III trial demonstrated that sacubitril/valsartan 97/103 mg twice daily can be safely used in patients with eGFR 20-60 mL/min/1.73 m², showing similar effects on kidney function compared to irbesartan while providing additional benefits in blood pressure and cardiac biomarker reduction 2. Recent real-world evidence confirms that the full dose of 97/103 mg twice daily is both effective and safe in CKD stages 3-5, with significant improvements in cardiac function and preservation of renal function 3.
Critical Monitoring Requirements
Within 2-4 weeks of initiation or dose increase, check: 4, 5, 4
- Serum creatinine and eGFR
- Serum potassium
- Blood pressure
Continue therapy unless: 4, 5, 4
- Serum creatinine rises >30% within 4 weeks
- Symptomatic hypotension occurs
- Uncontrolled hyperkalemia develops despite medical management
Common Pitfalls to Avoid
Do not unnecessarily limit the dose in CKD patients. While starting at a lower dose in severe renal impairment is appropriate, the goal should still be to reach the maximum tolerated dose of 97/103 mg twice daily, as this is where the proven cardiovascular and renal benefits were demonstrated in clinical trials 5, 4, 5.
Do not discontinue for mild eGFR decreases. A reversible decline in eGFR upon initiation is expected and is not an indication to stop therapy unless creatinine rises >30% within 4 weeks 4.
Manage hyperkalemia proactively rather than reducing dose. Hyperkalemia can often be managed with dietary modifications, potassium binders, or adjustments to other medications rather than reducing or stopping the RAS inhibitor component 5, 4, 5.
Special Populations
Dialysis patients: Case reports demonstrate feasibility of moderate doses (49/51 mg twice daily) in hemodialysis patients, though this is off-label and requires careful monitoring 6. Peritoneal dialysis patients have been safely treated with up to 100 mg twice daily with good tolerability 7.
