Heparin Drip Should Be Discontinued After PCI with Stent Placement and Dual Antiplatelet Loading
The heparin infusion can and should be stopped immediately after the coronary intervention is complete. Once the patient has received loading doses of ticagrelor 180 mg and aspirin 325 mg following successful stent placement, there is no indication to continue parenteral anticoagulation.
Rationale for Discontinuing Heparin
Discontinuation of parenteral anticoagulation should be considered immediately after an invasive procedure 1. The 2018 ESC/EACTS guidelines explicitly recommend this practice, and the 2025 ACC/AHA guidelines support anticoagulation during PCI but do not recommend continuation post-procedure 2, 1.
Key Supporting Evidence:
Anticoagulation is recommended for all patients during PCI as an adjunct to antiplatelet therapy, but this is specifically for the procedural period 1
Routine postprocedural heparin infusion is not recommended in patients after uncomplicated PCI 3
The patient has already received appropriate dual antiplatelet therapy with ticagrelor 180 mg loading dose and aspirin 325 mg, which provides the necessary ongoing antithrombotic protection 2, 4
Post-PCI Antithrombotic Management
The patient should continue on dual antiplatelet therapy (DAPT) alone without any parenteral anticoagulation 2, 1. The appropriate regimen going forward is:
Ticagrelor 90 mg twice daily (first maintenance dose 6-12 hours after the 180 mg loading dose) 2, 4
Aspirin 75-100 mg daily for maintenance (the 2025 ACC/AHA guidelines specifically recommend this lower maintenance dose rather than continuing 325 mg) 2
DAPT should be continued for 12 months after ACS with stent placement unless there are contraindications such as excessive bleeding risk 2, 1
Important Caveats
The only scenario where heparin might be continued would be if there were procedural complications such as no-reflow, thrombotic complications, or if the patient has another independent indication for anticoagulation (such as atrial fibrillation, mechanical valve, or venous thromboembolism) 1.
Combining heparin with DAPT significantly increases bleeding risk without providing additional benefit in uncomplicated cases 3, 5. One case report documented diffuse alveolar hemorrhage when LMWH was combined with ticagrelor and aspirin after PCI 5.
Aspirin dose matters with ticagrelor: The maintenance aspirin dose should be kept at ≤100 mg daily when used with ticagrelor, as higher doses may reduce ticagrelor's efficacy 2.
