Modified Ryan Tumor Regression Grading System
The modified Ryan tumor regression grading system is a 4-tier pathological scoring system (Grades 0-3) used to assess treatment response in esophageal cancer after neoadjuvant chemoradiotherapy, based on the amount of residual viable cancer cells present in the resection specimen. 1
Grading Scale and Interpretation
The modified Ryan system categorizes tumor regression as follows:
Grade 0 (Complete Response): No residual cancer cells present 1
Grade 1 (Moderate Response): Single cells or small groups of cancer cells remaining 1
Grade 2 (Minimal Response): Residual cancer cells outgrown by fibrosis 1
Grade 3 (Poor Response): Minimum or no treatment effect with extensive residual cancer 1
Clinical Application and Prognostic Significance
The modified Ryan system should be reported in all post-neoadjuvant esophagectomy specimens, as residual primary tumor after neoadjuvant therapy is associated with shorter overall survival in both adenocarcinoma and squamous cell carcinoma of the esophagus. 1
Key Assessment Principles:
The entire esophagogastric junction (EGJ) or ulcer bed should be thoroughly sampled and submitted for specimens following neoadjuvant therapy without grossly obvious residual tumor 1
Sizable pools of acellular mucin may be present after chemoradiation but should NOT be interpreted as representing residual tumor 1
The system provides good reproducibility among pathologists, with 3-category systems generally showing excellent interobserver agreement 1
Important Caveats and Limitations
Recent validation studies have revealed conflicting data regarding the prognostic significance of the modified Ryan system. While NCCN guidelines recommend its use 1, a 2024 study found that the modified Ryan system did NOT correlate with disease-free survival in esophageal cancer patients, despite showing substantial inter-rater agreement (kappa 0.774) 2. This same study found that only the modified rectal cancer regression grading (mRCRG) system showed prognostic significance 2.
Critical Distinction from Other Systems:
The modified Ryan system differs fundamentally from other tumor regression grading systems (Mandard, Dworak, AJCC) because it categorizes tumor regression based solely on the quantity of residual viable cancer cells, NOT on the relationship between residual tumor and therapy-induced fibrosis 3. This results in lower correlation coefficient values (0.549-0.617) when compared to fibrosis-based systems 3.
Alternative Systems to Consider
While the modified Ryan system remains recommended by NCCN 1, pathologists may also use:
The Wu et al. 3-tiered system (0%, 1-50%, >50% residual cancer), which is reported to provide good interobserver agreement 1
The CAP Cancer Protocol system for esophageal carcinoma 1
Systems that incorporate both residual tumor and fibrosis assessment show stronger correlation with each other (correlation coefficients 0.964-1.0) 3
