Injection Site Performance of Zepbound (Tirzepatide)
The efficacy of Zepbound does not differ based on subcutaneous injection site—all recommended sites (abdomen, thigh, buttock, and upper arm) are equally effective when proper subcutaneous technique is used. 1
Recommended Injection Sites
Zepbound should be administered subcutaneously into any of the following sites 1:
- Abdomen
- Thigh
- Buttock
- Upper arm
Critical Technical Considerations
Avoid Intramuscular Injection
The most important factor affecting tirzepatide performance is ensuring subcutaneous rather than intramuscular delivery 1:
- Intramuscular injection leads to unpredictable absorption and variable glucose effects 1
- IM delivery is associated with frequent and unexplained hypoglycemia 1
- Risk of inadvertent IM injection increases with:
Needle Length Recommendation
Use short needles (4-mm pen needles) to minimize risk of intramuscular injection—these are effective and well-tolerated even in adults with obesity 1
Site Rotation Protocol
Rotate injection sites systematically to prevent lipohypertrophy 1:
- Lipohypertrophy causes erratic insulin absorption, increased glycemic variability, and unexplained hypoglycemia 1
- Appears as soft, smooth raised areas several centimeters in breadth 1
- Never inject into areas of lipohypertrophy 1
- Examine injection sites regularly for these changes 1
Immunogenicity Considerations
While injection site reactions can occur, they do not affect drug efficacy 2:
- Treatment-emergent antidrug antibodies developed in 51.1% of tirzepatide-treated patients 2
- More patients with antibodies experienced injection site reactions compared to those without 2
- However, these reactions were predominantly mild to moderate and did not affect pharmacokinetics or efficacy 2
- Antibody status, titer, and neutralizing antibody presence had no effect on drug performance 2
Practical Algorithm
When administering Zepbound:
- Select any of the four approved sites (no performance difference between sites) 1
- Use 4-mm needles to ensure subcutaneous delivery 1
- Prefer truncal sites (abdomen, buttocks) in leaner patients to reduce IM risk 1
- Rotate sites systematically within and between anatomical areas 1
- Inspect sites before each injection and avoid any areas with lipohypertrophy 1
