Elevated Serum Iron with Normal Ferritin: Interpretation
An elevated serum iron level with normal ferritin is most commonly a transient laboratory finding that does not indicate iron overload and typically requires no immediate intervention beyond repeat testing and evaluation for secondary causes. 1
Key Diagnostic Principle
- Transferrin saturation (TSAT), not isolated serum iron, is the critical screening parameter for iron overload. 2, 3
- Serum iron concentration alone does not quantitatively reflect body iron stores and should not be used as a surrogate marker of tissue iron overload. 3
- Normal ferritin essentially rules out iron overload, as ferritin is a highly sensitive test for iron stores when inflammation is absent. 3, 4
Pathophysiologic Context
- True iron overload presents first with elevated TSAT (>45-50%), followed later by rising ferritin. 1
- In hereditary hemochromatosis, elevated TSAT is the earliest laboratory abnormality; isolated ferritin elevation without TSAT rise is atypical. 1
- Serum iron fluctuates significantly with diurnal variation, recent dietary intake, and acute inflammation, making single measurements unreliable. 1
Recommended Diagnostic Algorithm
Step 1: Calculate Transferrin Saturation
- Calculate TSAT = (serum iron ÷ total iron-binding capacity) × 100 using a fasting morning sample. 2, 3, 1
- If TSAT <45%, iron overload is effectively excluded and no further hemochromatosis work-up is needed. 2, 3, 1
- If TSAT ≥45% with normal ferritin, repeat testing after an overnight fast to confirm the finding before proceeding. 2, 1
Step 2: Evaluate for Secondary Causes of Elevated Serum Iron
When serum iron is elevated but ferritin and TSAT remain normal, consider:
- Recent oral or intravenous iron supplementation (can transiently elevate serum iron without affecting stores). 1
- Hemolytic anemia (releases iron from lysed red blood cells into circulation). 1
- Acute hepatocellular necrosis (releases stored iron from damaged hepatocytes). 3
- Laboratory error or non-fasting sample (most common explanation for isolated serum iron elevation). 2, 1
Step 3: Repeat Testing if Clinically Indicated
- Recheck fasting serum iron, TSAT, and ferritin in 2-4 weeks if initial values are discordant. 1
- Persistent elevation of serum iron with normal TSAT and ferritin over serial measurements is extremely rare and warrants hematology consultation. 1
When to Pursue Hemochromatosis Evaluation
HFE genetic testing is indicated only when ALL of the following criteria are met: 2, 3, 1
- TSAT ≥45% on repeat fasting measurement 2, 3, 1
- Ferritin >300 μg/L in men or >200 μg/L in women 2, 3, 1
- No clear inflammatory, hepatic, or secondary cause identified 3, 1
Common Pitfalls to Avoid
- Do not order HFE genetic testing based on isolated serum iron elevation without elevated TSAT. 1
- Do not assume iron overload exists when ferritin is normal, regardless of serum iron level. 3, 1, 4
- Do not initiate therapeutic phlebotomy without documented elevation of both TSAT and ferritin. 2, 5, 1
- Do not overlook that approximately 90% of hyperferritinemia cases are due to inflammation, liver disease, or metabolic syndrome rather than true iron overload. 3, 5
Clinical Bottom Line
- Normal ferritin with elevated serum iron is almost never clinically significant and does not represent iron overload. 1, 4
- TSAT is the gatekeeper test: if normal, iron overload is excluded regardless of serum iron level. 2, 3, 1
- Repeat fasting measurements and evaluate for secondary causes before pursuing expensive or invasive testing. 2, 1
