Methylphenidate ER 30mg Biphasic Capsule Administration
Do not split, cut, or crush methylphenidate extended-release biphasic capsules; however, the capsule may be opened and the entire contents sprinkled on applesauce for immediate consumption if swallowing the capsule whole is difficult. 1, 2
Administration Guidelines
Intact Capsule Administration
- Swallow the capsule whole with water or other liquid 1
- Take preferably 30 to 45 minutes before meals 3
- The biphasic formulation is designed to provide immediate release of 30% of the dose followed by extended release of 70%, mimicking two separate doses given hours apart 4
Alternative Administration for Swallowing Difficulties
- The capsule may be opened and the microgranule beads inside can be sprinkled on a small amount of applesauce 1, 2
- Consume the entire applesauce mixture immediately without chewing the beads 2
- Do not store the applesauce-medication mixture for later use 2
- This method preserves the extended-release properties of the beads 2
Critical Contraindications
What NOT to Do
- Never cut, crush, or chew the capsule or beads as this destroys the extended-release mechanism and can cause dose dumping 1
- Crushing or chewing converts the entire 30mg dose to immediate release, creating risk of:
Alcohol Interaction Warning
- Avoid alcohol consumption with extended-release methylphenidate formulations 5
- In vitro data suggest some extended-release stimulants undergo dose dumping when exposed to alcohol, releasing a dangerous amount of medication rapidly 5
Timing Considerations
- For patients with sleep difficulties, administer the last dose before 6 p.m. 3
- The biphasic formulation provides symptom control throughout the day (8-12 hours) with once-daily dosing 2, 4
Common Pitfall
The most common error is crushing or chewing the medication to facilitate administration—this completely negates the extended-release design and creates safety risks. If a patient cannot swallow capsules, opening and sprinkling on applesauce is the only acceptable alternative that maintains the medication's intended pharmacokinetic profile 1, 2.
