Enalapril Dosing Guidelines
Start enalapril at 5 mg once daily for hypertension in patients not on diuretics, 2.5 mg once daily for heart failure, and reduce initial doses to 2.5 mg daily in patients with renal impairment (CrCl ≤30 mL/min), elderly patients, or those on concurrent diuretics. 1
Hypertension Dosing
Standard Initiation
- Initial dose: 5 mg once daily in patients not currently on diuretics 1
- Maintenance range: 10-40 mg daily, administered as a single dose or divided into two doses 1
- If antihypertensive effect diminishes toward the end of the dosing interval with once-daily administration, consider increasing the dose or switching to twice-daily dosing 1
Patients Currently on Diuretics
- Discontinue diuretic 2-3 days before starting enalapril if possible to reduce hypotension risk 1
- If diuretic cannot be discontinued, start with 2.5 mg enalapril under medical supervision for at least 2 hours and until blood pressure stabilizes for an additional hour 1
- Diuretic therapy may be resumed if blood pressure is not controlled with enalapril alone 1
Renal Impairment in Hypertension
- Normal renal function (CrCl >80 mL/min): 5 mg daily 1
- Mild impairment (CrCl 30-80 mL/min): 5 mg daily 1
- Moderate to severe impairment (CrCl ≤30 mL/min or serum creatinine ≥3 mg/dL): 2.5 mg once daily, titrate upward to maximum 40 mg daily as tolerated 1
- Dialysis patients: 2.5 mg on dialysis days, adjust dosing on non-dialysis days based on blood pressure response 1
Research supports that enalapril 5-10 mg daily is effective in lowering blood pressure in patients with chronic renal insufficiency without significant changes in renal function parameters 2
Heart Failure Dosing
Standard Heart Failure Initiation
- Initial dose: 2.5 mg once or twice daily 1
- Target dosing range: 2.5-20 mg twice daily (recommended dosing range) 1
- Maximum dose: 40 mg daily in divided doses (maximum tested in clinical trials) 1
- Titrate upward as tolerated over a period of days to weeks 1
- The landmark CONSENSUS trial demonstrated mortality reduction with enalapril 2.5-40 mg daily in severe heart failure (NYHA Class IV), with a 40% reduction in 6-month mortality 3
Heart Failure with Renal Impairment or Hyponatremia
- Initiate at 2.5 mg daily in patients with serum sodium <130 mEq/L or serum creatinine >1.6 mg/dL 1
- Titrate cautiously: increase to 2.5 mg twice daily, then 5 mg twice daily, and higher as needed at intervals of 4 days or more 1
- Monitor closely for hypotension and renal function deterioration 1
- Maximum daily dose remains 40 mg 1
Monitoring After Initial Dose
- Observe patient under medical supervision for at least 2 hours after the first dose and until blood pressure stabilizes for an additional hour 1
- Reduce concomitant diuretic dose if possible to minimize hypotension risk 1
- First-dose hypotension does not preclude subsequent careful dose titration after effective hypotension management 1
Special Populations
Elderly Patients
- Use caution with initial dosing; consider starting at lower end of dosing range 4
- In very elderly patients (mean age 76 years) with rapidly progressive heart failure, enalapril can be safely administered if baseline serum creatinine is <1.9 mg/dL 5
- Patients with serum creatinine ≥1.9 mg/dL require careful monitoring and prompt discontinuation if renal function deteriorates 5
- Renal damage in elderly patients typically occurs within the first 4 days and is reversible with enalapril discontinuation 5
Chronic Kidney Disease
- Evidence from the SOLVD trial demonstrates that enalapril reduces mortality and cardiovascular hospitalization in systolic heart failure patients both with and without CKD (eGFR <60 mL/min/1.73 m²), with no significant heterogeneity in treatment effect 6
- Avoid use in neonates and pediatric patients with GFR <30 mL/min/1.73 m² due to lack of data 1
Dose-Response Considerations
Titration to Target Doses
- Higher doses (40 mg daily) provide superior outcomes compared to lower doses (10 mg daily) in heart failure patients, with improved exercise capacity (maximal oxygen consumption) and reduced neurohormonal stimulation (ANP, BNP, norepinephrine) 7
- The most commonly effective doses in heart failure are 10-20 mg daily, administered once or twice daily 8
- Clinical trials demonstrating mortality benefit in heart failure used target doses of 10 mg twice daily (20 mg total daily) 9
Critical Safety Considerations
Monitoring Requirements
- Monitor blood pressure closely during initiation and titration, especially in volume-depleted patients 1
- Monitor serum potassium as enalapril can increase potassium levels; avoid concomitant use with potassium supplements, potassium salt substitutes, or potassium-sparing diuretics 1
- Monitor renal function, particularly in patients with baseline renal impairment or bilateral renal artery stenosis 4
Contraindications and Precautions
- Do not administer within 36 hours of switching from or to an ARNI (angiotensin receptor-neprilysin inhibitor) due to angioedema risk 9
- Patients intolerant to ACE inhibitors due to cough or angioedema should be switched to an ARB, though caution is advised as some patients develop angioedema with ARBs as well 9
- For patients with stable HFrEF who tolerate ACE inhibitors, consider switching to ARNI (sacubitril-valsartan) for superior mortality and hospitalization reduction 9
